Adult Strabismus Surgery DFW

Adults who notice their eyes drifting out of alignment, whether from a childhood condition that resurfaced or something that developed later in life, often assume surgery is a childhood-only fix. It isn't.

Strabismus surgery for adults is a well-established procedure that adjusts the muscles responsible for eye movement, correcting the visible turn and often easing the double vision that tends to come with it.

Knowing what the procedure actually involves, from how surgeons choose which muscles to work on to what recovery generally looks like, makes it easier to decide whether this is the right next step.

ABOUT US

Strabismus surgery repositions one or more of the six muscles that control how the eye moves, either by shortening a muscle, lengthening it, or reattaching it at a new point on the eye's surface.

The surgeon selects which muscles to adjust based on the direction and degree of the misalignment, along with how the eye behaves when looking in different directions.

Adults are typically given a choice between general anesthesia and local anesthesia with sedation, and the procedure is done on an outpatient basis. Most cases involve one or two muscles per eye, though more complex misalignments can require adjustments to both eyes in the same visit.

Before the procedure, the surgical team measures the exact angle of misalignment in several directions of gaze, and those measurements become the basis for calculating how far each muscle needs to move. The eye itself stays in place throughout, since the surgeon works on the muscle tissue surrounding it rather than removing or replacing anything from the eye.

Adult-onset strabismus usually traces back to one of a few causes: a childhood misalignment that was never fully corrected and slowly worsened, a stroke or nerve condition affecting muscle control, thyroid eye disease, or the general weakening of eye muscles that can accompany aging. Unlike children, whose developing brains often adapt to a misalignment by suppressing input from one eye, adults typically experience persistent double vision because that adaptive process no longer happens as readily.

This is part of why adult strabismus often gets diagnosed only after double vision becomes disruptive enough to prompt an eye exam, rather than being caught earlier through a routine screening.

Vision changes tied to cataracts or other age-related eye conditions can also shift the balance between the two eyes over time, contributing to a misalignment that wasn't noticeable in earlier years. In cases connected to a stroke or nerve issue, the onset is often sudden rather than gradual, which tends to prompt a faster trip to the eye doctor.

Most strabismus procedures for adults take less than an hour in the operating room, though the exact time depends on how many muscles need adjustment and whether both eyes are involved. It's performed as outpatient surgery, meaning patients go home the same day rather than staying overnight.

Someone else needs to handle the drive home, since vision is often blurry and eyes may be sensitive to light immediately afterward. The appointment itself, including pre-op preparation and a short recovery period before discharge, generally takes a few hours from arrival to walking out the door.

Anesthesia recovery time gets factored in separately from the surgical time itself, since the surgical center wants to confirm you're stable and alert before releasing you. Patients having both eyes worked on in the same visit should expect the total appointment window to run somewhat longer than a single-eye procedure.

Many adults notice a visibly straighter eye alignment within days of surgery, though some redness, swelling, mild discomfort, and light sensitivity are normal for the first couple of weeks. Double vision, when present before surgery, often improves gradually as the brain adjusts to the new alignment rather than resolving instantly on the operating table.

Results vary depending on the underlying cause and how long the misalignment was present before treatment, and some adults need a secondary adjustment months later to fine-tune the outcome.

A follow-up exam schedule helps the surgical team track how the eyes are settling and catch anything that needs attention early.
