Request My Report

If you would like to request a copy of your recently submitted IME case in PDF, please complete the below form, and it will be generated and emailed to you by our office.  Make sure to wait 10 days after your exam before this request. Alternatively, you can request it in person.

Note that this is only available for cases processed since Jan 1, 2026. 

Do not attempt to retrieve a report for a case older than 12 months.
