Congratulations on completing your course or program.
After you and your teacher have checked off competencies in DACESIS and you have been marked in as a completer for the class, please fill out the form below to request your certificate of completion:
Be sure to list all of the courses for which you are requesting a Certificate of Completion on the form.
Please follow up with the staff below if you haven't received your Certificate of Completion in about a week after submitting the form:
