Long Term Care Council

of Warren, Hamilton, and Washington Counties


NY Connects Long Term Care Council (LTCC) is made up of individuals receiving services, caregivers, health care providers, community members and many agencies who work together to problem solve issues related to those with long term care needs. The LTCC assesses needs, identifies gaps in long term care and works to improve service delivery for all consumers.

The LTCC meets quarterly in various locations throughout our region and the Interagency Council does not meet on those months. Please check the Interagency calendar for the next LTCC meeting date. For more information about LTCC email ltcc@adkinteragency.org.

The LTCC is affiliated with NY Connects which is a statewide, locally based, "No Wrong Door" entry system providing one stop access to free, objective, and comprehensive information and assistance related to long term care services and supports.
