Originally given as an in-person presentation by Adrienne Doucette and Denise Weeks, the ACP+D presentation is now a recorded presentation done by Denise that you can watch online. It came about from the encouragement and help of Hilary Walker, Advance Care Planning Coordinator at PeaceHealth. Additional support came from Sharmon Figenshaw, ARNP, Hospice and Palliative Care Specialist, & POLST Task Force.

To date, we have given many presentations to the Whatcom County community on the importance of Advance Care Planning for Dementia, including a presentation at the October 2018 Alzheimer Society Conference in Bellingham, Washington, and at the Washington State Medical Association's November 2019 End of Life Care Coalition conference.

Adrienne Doucette is an educator, a trained Advance Care Planning facilitator, and a Hospice volunteer. She brings her expertise and compassion to help adults think through their end-of-life options and complete their Advance Directives. She is passionate about the important process of making ones wishes known at the end of life, especially when there is a diagnosis of dementia.

Denise Weeks received training as an Advance Care Planning facilitator through the Whatcom Alliance for Health Advancement (WAHA). She is the author of Mantra: Repetition in the Land of Alzheimer’s, about giving care to her mother with Alzheimer’s disease, and she is co-leader of a local support group for adult children caring for a parent with dementia.
