CARDIOVASCULAR CASES
A few of the Cases I came across during the Internship were :
C/O - Acute Kidney Injury [AKI] on Chronic Kidney Disease [CKD] on Hemodialysis [HD], Pyelonephritis, HTN, T2DM
C/O - Bilateral Basal consolidation, Left Distal End of Radius Fraction, Pulmonary Contusion, Aspiration Pneumonitis
C/O - HTN, Heart Failure, T2DM, OHA induced Hypoglycemia, AKI on HD
C/O - Posterior, Fossa Decompression
C/O - Exploratory Laparotomy with excision of Right Ovarian Mass with excision biopsy of para gastric lymph node
C/O - RTA, Left IC Bleed, Grade 4 Liver Failure
CASE SUMMARY OF ONE OF THE CASES DURING CVRS SICU POSTING :
Name : Uttam Agarwal
Age / Gender : 54 / Male
Diagnosis : Grade 4 Liver injury, left IC bleed
HOPI : Patient was apparently alright before 25th June 2025 when on 26th June evening he was on a two wheeler driving on the main road and a dog ran passed from the patient's right side causing him to loose his balance and crashed into the footpath on left side arm, forearm and hand causing abrasions on the upper limb along with hitting his head on the footpath and the Bike handle hitting his abdominal right side area causing injury to his liver.
Investigations : Rasiological findings showed that there were Homogeneous opacities visible more medially in all 3 zones i.e, upper, middle, and lower initially. Later over time it cleared due to the physiotherapy protocol administered to the patient.
On Auscultation : Initially the air entry was bilaterally reduced in all 3 zones, eventually cleared with findings being air entry bilaterally equal in all 3 zones.
Management : Initially patient was receiving Passive ROM ex and as he was on ventilator, breathing exercises were not possible for the chest so Percussions, Vibrations, Postural Drainage, Positioning were given. Over time as and when his condition improved, patient was given the following protocol :
Pursed lip breathing exercise
Diaphragmatic breathing exercise
Segmental breathing exercise
Thoracic breathing exercise
End expiratory stretches
Non affected side of the body was administered with AROM Upper limb and Lower limb exercises
Ending with coming to the edge of the bed sitting position with support. In the ICU.
NOTE : When the patient was shifted from SICU to the surgery HDU as his condition got better, there he got a mucus plug in this suction tube while clearing the Tracheostomy Tube leading to blockage of the airway, leading to sudden rapid drop in the vitals. Due to which CPCR had to be administered that went on for around 15 mins. This lead to deterioration in the patient's progress where in he was not able to be responsive to any external stimulus except for pain when the sternum was rubbed forcefully. [ E1, VT, M1 ] [ no verbal response, intubated ].
REFLECTION REGARDING THE ABOVE CASE :
Going through this experience taught me that one can never be too careful. Its always the best and safest option to check recheck anything you do, make sure all protocols, rules, steps are followed especially in the medical profession as even though we as physiotherapist do not cut the patient with a scalpel like a surgeon but we can cause even more harm to the patient where our one mistake can effect another human's life drastically, leaving them in a vegetative, completely dependent form which is even more troublesome and tough to deal with. If we have given certain instructions to the nursing stuff for patient care in our absence, it is our responsibility even then to check recheck with them if they have followed everything in a safe and proper manner by perhaps calling them asking for an update. A Doctor is always on Call. It taught me to always be vigilant, observe everything in my surrounding, ask for help when need be, check everything every detail about the patient before touching the patient, to always make sure all lines and tubes attached to the patient are untangled, not bent, have a clear passage and loose enough that they don't get detached from the patient or the power source. It was a human mistake, but precaution is always better than cure.

CASE SUMMARY OF ONE OF THE CASES DURING BELAPUR MGM HOSPITAL
OPD POSTING
Name : Surji Khaur
Age / Gender : 65 / Female
Diagnosis : Peri Arthritis Shoulder / Frozen Shoulder
HOPI : Patient was apparently alright till the month of August, when on September 1st she started having pain in her left shoulder, neck, upper back area. Patient had been doing repetitive movements in the kitchen without any rest pauses or pacing and had also slept in an awkward, improper position on a wrong type of pillow. This eventually led to pain, restricted movements, swelling in the affected area and triggered Frozen shoulder condition also one of the reasons being that she has had diabetes for past 20 years. Since past one month since 6th September patient has been coming to the Belapur MGM Hospital, Physiotherapy clinic for the physiotherapy treatment protocol.
On Observation : Swelling present over the left shoulder area, continuing in the upper arm region.
On Examination : Warmth present, tenderness present, swelling / inflammation present. No scars or open wounds present.
On Assessment : Restricted and reduced AROM of the all shoulder joint actions. Reduced Muscle strength.
Management : Initially the patient was advised to do cryotherapy at home using an ice pack keeping it for 10 to 15 mins at every area that felt painful and swollen. In the OPD the patient was given Transcutaneous Electrical Nerve Stimulation [ TENS ] to manage the pain, lead by Myofascial Release [ MFR ] to release the fascia and the soft tissue in the area. Lead by Manually Trigger point release. Rest pause, few breathing exercises then leading Maitland Mobilization [ Grade 1] then to Free AROM exercises in gravity eliminated plane initially then against gravity plane. Stretching and strengthening of all the cervical, scapular, rotator cuff, arm, forearm, wrist and finger and core muscles. Initially Freely > then against manual mild resistance > then stronger manual resistance and then slowly introducing light weights like with Dumbbell and Weight cuffs.

METHEDOLOGY FOR MY RESEARCH
• Approval was obtained from the Institutional Ethical Committee.
• After screening, 384 was selected on the basis of inclusion and exclusion criteria.
• Permission Letter to collect data and click pictures was collected from the college with college letter head and submitted to the HOD of the respected area BMC offices .
• The HODs gave me a NOC [ No objection certificate ] letter after explaining them about my study .
• The HODs Instructed then Motor Loader Manager who manages different wards / areas and multiple groups of motor loaders to
• Appointed a Mukadam to me, who goes along with the motor loaders to keep a check on them and cater to their problems faced on site , he guided me through the directions as to where the particular waste collection containers are and waste collection sites.
• Procedure and Purpose of the study was explained to the participants.
• Informed consent was taken of those participants willing to participate voluntarily. Confidentiality was ensured. Pictures were taken from a distance while they did their work.
• All the participants were subjected to NMQ, QEC ,OSI , IPAQ – SHORT FORM, REBA, Phone Camera, Kinovea Soft - wear , Chalder Fatigue Scale [ CFS ], Consent form and Case Record form.
• All the Data was tabulated in MS – EXCEL and analysed further, Master chart was made and the Statistical analysis was done using SPSS software.
