POC Feedback Form
Use this form to leave feedback for POC cadets. This feedback may be filled out by GMC/POC alike. Additionally, this form may be praise or constructive by nature, I just ask that you remember to KEEP IT PROFESSIONAL!
DO NOT LEAVE ONE LINER FEEDBACKS!
There are many models to leaving effective feedback. If you need help getting started, check out this example:
A. State the specific event, action or behavior
B. State the impact of that behavior
C. Start with “I like” to encourage or “I wish” to change a person’s specific behavior
Example using the 3 parts:
“When you came late to the meeting this morning [A], the whole team had to wait before we could start [B]. I wish you would arrive earlier in the future so that the meeting can start on time [C].” Source
