The polymicrobial nature of diabetic foot infection (DFI) makes accurate identification of the DFI microbiota, including rapid detection of drug resistance, challenging. Therefore, the main objective of this study was to apply matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI TOF MS) technique accompanied by multiply culture conditions to determine the microbial patterns of DFIs, as well as to assess the occurrence of drug resistance among Gram-negative bacterial isolates considered a significant cause of the multidrug resistance spread. Furthermore, the results were compared with those obtained using molecular techniques (16S rDNA sequencing, multiplex PCR targeting drug resistance genes) and conventional antibiotic resistance detection methods (Etest strips). The applied MALDI-based method revealed that, by far, most of the infections were polymicrobial (97%) and involved many Gram-positive and -negative bacterial species-19 genera and 16 families in total, mostly Enterobacteriaceae (24.3%), Staphylococcaceae (20.7%), and Enterococcaceae (19.8%). MALDI drug-resistance assay was characterized by higher rate of extended-spectrum beta-lactamases (ESBLs) and carbapenemases producers compared to the reference methods (respectively 31% and 10% compared to 21% and 2%) and revealed that both the incidence of drug resistance and the species composition of DFI were dependent on the antibiotic therapy used. MALDI approach included antibiotic resistance assay and multiply culture conditions provides microbial identification at the level of DNA sequencing, allow isolation of both common (eg. Enterococcus faecalis) and rare (such as Myroides odoratimimus) bacterial species, and is effective in detecting antibiotic-resistance, especially those of particular interest-ESBLs and carbapenemases.

Definition: Hand-foot skin reaction (HFSR) refers to symptoms affecting the hands and/or feet associated with multikinase inhibitors.1

Hand-foot skin reaction is distinct from the hand-foot syndrome (HFS). HFS, also known as palmar-plantar erythrodysesthesia, is associated with chemotherapy agents such as 5-fluorouracil and capecitabine.1 HFS presents with diffuse painful oedema and redness of palms and soles. Whereas HFSR is dose-dependent and characteristically localizes to areas of pressure or friction on the skin, such as on the heels, metatarsal heads, and areas of friction caused by shoes or manual labour. Lesions are sharply demarcated, erythematous, oedematous, painful and very tender blisters that evolve into inflamed and painful calluses.2,3

Resolution: Symptoms of HFSR may diminish as treatment continues and usually it can be successfully controlled with appropriate management.18-23 Also see Prophylaxis and treatment - reactive management - Hand-foot Skin Reaction page.

The NCI-CTCAE v5.0 does not grade HFSR but definition for grade 1 Palmar-plantar erythrodysesthesia syndrome: Minimal skin changes or dermatitis (e.g., erythema, oedema, or hyperkeratosis) without pain and not limiting instrumental ADL.

The corresponding NCI-CTCAE v5.0 definition for grade 2 Palmar-plantar erythrodysesthesia syndrome states: skin changes (e.g., peeling, blisters, bleeding, oedema, or hyperkeratosis) with pain; slightly limiting instrumental ADL.

Apple was one of the first consumer hardware companies to consistently insist on reducing or removing visible fasteners on their products, and this computer is no exception. With just a single split line hidden underneath the main body of the device, it was daunting at first glance to see how to open the product. But underneath a single molded plastic ring affixed with adhesive, four large Torx screws hold the machined aluminum foot to the rest of the computer.

The foot itself is quite interesting with its precisely machined hole pattern of more than 1500 holes drilled at a 45 degree slope. In addition, a seamlessly integrated plastic ring on the bead blasted surface surrounds the entire part. Taking a closer look with the Instrumental platform, we focused on a couple of interesting nuggets that might have slipped past other reviewers.

After removing the bottom foot of the device we are immediately greeted with a large circular circuit board with a matte black plastic cover. Also visible are the three main antennas on the perimeter of the product which helps explain why the designers went with a plastic perimeter on the aluminum foot. Held on by another 6 screws and two connectors it turns out to be the main power supply of the product. Tobias explored the board using the Instrumental platform while I removed some of the many screws in the product to save time. The black plastic cover is both an aesthetic choice and a way to insulate the high power components from the metal chassis. Large swathes of black epoxy help isolate components from one another to prevent shorting.

Tobias and I had a lot of fun with this teardown because there were so many different things to geek out on. There are several problems that principles of instrumentaltransmission need to avoid. First, means to an end can becostly, or otherwise objectionable. Suppose that I ought totake a job in Alaska. One (sufficient) means is to fly there; another(sufficient) means is to take free passage on a merchant ship. Butflying costs more than I would earn there. Presumably, I ought notfly. It may be said that I have some reason to fly, but that it isoutweighed. This is an argument against Ought Sufficiency.

Now we consider instrumental rationality understood as a matter ofrational coherence. It is often said that someone is being locallyinstrumentally incoherent insofar as he intends an end, believes,roughly, that something is a means to it, but refuses to intend thatmeans. More precisely, it seems that this is so only when the relevantbelief is about necessary means. Suppose that I intend to goto Alaska, but believe that flying there is a sufficient, but notnecessary, means to going to Alaska. Am I being instrumentallyincoherent in refusing to intend to fly there? Not obviously: perhapsI believe that going by sea is cheaper, and I intend that.

Moreover, the relevant belief must be that intending the meansnow is necessary. Suppose a beginning student intends to earna doctoral degree and believes that, when the dissertation iscompleted, intending to submit the final paperwork is necessary forearning the degree. It is not clear that she is being instrumentallyincoherent in refusing to intend, now, to submit that paperwork. Shemay trust that, when the time comes, she will intend it (Setiya 2007).This suggests, as Raz notes, that opportunities for instrumentalincoherence may actually be very rare.

There are, of course, other answers that could be given. (Forinstance, we briefly discuss a constitutivist approach to instrumentalincoherence, attributed to Kant, in 4.4) But consideration of these approaches will give a reasonablycomprehensive picture of the recent attempts to explain the ViolationClaim.

Certainly, this can happen in some cases, at least accidentally.Suppose our madman intends to start World War III, believes that hewill start World War III only if he intends to launch a nuclearmissile, but refuses to intend to launch a nuclear missile. TheViolation Claim might be explained in this case by pointing out that,whatever his other attitudes may be, he ought not to have theintention to start World War III, but does have that intention.
