本人因參加財團法人李長榮教育基金會(以下簡稱「基金會」)所舉辦之活動或研討會(以下簡稱「活動」)，同意提供本人個人資料（包含嗣後隨時依基金會之通知，提出因上述活動所需之相關文件或資訊），供基金會蒐集、處理及使用本人個人資料，用於該特定範圍內及活動相關。

I participate in the seminar or the activity (hereinafter referred to as this “Activity”) holding by LCY Education Foundation (hereinafter referred to as the “Foundation”). I agree to provide my personal information (including providing relevant documents or information required for the Activity at any time in accordance with the notice of the Foundation.) to the Foundation for collection, processing or use within that specific scope and Activity.

本人同意基金會將本人之個人資料提供給基金會、李長榮化學工業股份有限公司、李長榮科技股份有限公司 (以下統稱「李長榮集團」）、其他與本基金會有業務往來之機構（包含但不限於金融或保險機構等）、依法有調查權機關或金融監理機關處理及利用。

I agree that Foundation could provide my personal information to the Foundation, LCY Chemical Corp., LCY Technology Corp. (collectively, “LCY Group”), and other institutions that have business dealings with the Foundation (including but not limited to financial or insurance institutions, etc.), agencies with investigative powers or financial supervisory agencies in accordance with the law will handle and use it.

活動期間，本人同意基金會可以就活動內容與過程進行錄影，並以照片、影片或動畫的方式記錄活動的過程；本人並同意授權基金會拍攝本人的照片、影像（統稱為肖像），並可以製作成影片，放在基金會網頁上，上開著作權及所有權均歸李長榮集團所有。

During the Activity, I agree that the Foundation can video record the content and process of the Activity and record the Activity process by taking photos, videos, or animations; I also agree to authorize the Foundation to take photos and images (collectively, portraits) of me, and may make it into a video and put it on the Foundation's website.  I agree that the copyright and ownership rights of the portraits photos, images and videos taken belong to LCY Group.

本人承諾所提供之個人資料為真實、完整且符合活動要求。若未提供詳實的個人資料，基金會得不同意受理或接受我的登記。

I promise that the Personal Information I provide is real, complete and match the need of this Activity. If I don’t provide my personal information, the Foundation don’t have to agree to handle it or accept my registration.

基金會之蒐集、處理及使用本人個人資料應遵循個人資料保護法及相關法令之規範，且本人有權就自身個人資料行使以下權利：(1)請求查詢或閱覽。(2)製給複製本。(3)請求補充或更正。(4)請求停止蒐集、處理及利用。(5)請求刪除。

When the Foundation collect, process or use my personal information, the Foundation should comply with the provisions of the Personal Data Protection Act and related laws and regulations. I have the right to exercise the following rights regarding my personal information:

(1) Request for inquiry or reading. (2) Make copy. (3) Request for supplement or correction. (4) Request to stop collection, processing and use.  (5) Request deletion.
