Chad perman, Mpp

Mr. Perman has served as the Executive Director of the Maryland Primary Care Program Management Office since September 2021. He previously served as the Program Director. On behalf of the Maryland Department of Health, Mr. Perman was a member of a small team that co-designed the innovative advanced primary care program with the Centers for Medicare and Medicaid Services from 2016 to 2017. He now oversees Maryland’s $200 million annual partnership with CMS, Medicare’s largest advanced primary care program in the Nation. Focused on integrating public health and primary care, Mr. Perman has been a key advisor to the Maryland Department of Health on health transformation and population health initiatives since 2015. Mr. Perman previously served as the Director of Health Systems Transformation within the Department’s Office of Population Health Improvement. Prior to working for the State, Mr. Perman served as a consultant with Health Management Associates (HMA). Mr. Perman conducted health policy analyses and provided consulting services to public- and private-sector clients focused on publicly financed health care. Mr. Perman has also presented at a variety of state and national meetings as well as authored several peer-reviewed publications. His abstract, “Innovative Payment Mechanisms in Maryland Hospitals" was selected as the 2014 Best of Academy Health Research Meeting. Mr. Perman leverages expertise in the areas of public policy analysis, consumer engagement, econometrics and performance measurement through a variety of engagements with federal agencies, universities and policy institutes. Mr. Perman has also served on a variety of boards and workgroups including the Herschel S. Horowitz Center for Health Literacy (University of Maryland) Advisory Board and the Maryland Primary Care Investment Workgroup. 

 

Raghavi Anand, Mph

Raghavi graduated from Johns Hopkins Bloomberg School of Public Health with a Master of Public Health and a certificate in Quality, Patient Safety, and Outcomes Research. She started with the Maryland Primary Care Program Management Office as a Healthcare Transformation Intern and was then hired on as the Quality and Learning Lead. Now, Raghavi is the Quality and Learning Manager at the Maryland Primary Care Program Management Office, where she manages the Quality Improvement and Learning work streams by spearheading quality improvement initiatives, creating strategies to support the program, and developing content for and managing the MDPCP Learning System. For 2023, Raghavi revamped the learning system to more intentionally focus on 3 core concepts: strategic goal alignment, fostering a culture of “all teach, all learn,” and support and sustainability. She has diligently worked to deliver this year’s Staff Training Academy, which is a testament to the revamp of the MDPCP Learning System. Raghavi will serve as a co-host on the day of the event— say hello! 

 

Ann greiner, mcp

Ann Greiner is the President and CEO of the Primary Care Collaborative (PCC), a role in which she champions comprehensive, team-based, and patient-centered primary care. With extensive experience in healthcare quality, Ann has held influential positions at organizations such as the National Quality Forum (NQF), the National Academies of Medicine, NCQA, and the American Board of Internal Medicine. She is highly regarded for her advocacy and leadership in advancing primary care. Ann holds a master's degree in Urban Planning from MIT and a Bachelor of Arts degree in English Literature from Hobart and William Smith Colleges.

 

Heidi Gullet, MD, MPH

Heidi Gullett, MD, MPH, is a dedicated healthcare professional and educator. She holds dual board certifications in Preventive Medicine and Family Medicine and practices at Neighborhood Family Practice in Cleveland. Dr. Gullett is actively involved in medical education, teaching students and residents while also serving in leadership roles. She is deeply committed to achieving health equity through her work as the population health liaison at the Cuyahoga County Board of Health and co-chairing the Health Improvement Partnership-Cuyahoga. With a focus on cancer prevention and poverty alleviation, Dr. Gullett's research embodies her passion for creating long-lasting partnerships and collective impact for the betterment of healthcare.

 

 

Karen twigg, MSM-HCA, BSN, RN, CMCN

Karen Twigg is the Vice President of Population Health, Community Health at The Coordinating Center and is responsible for oversight and direction of The Center’s work with hospitals, physician practices, managed care organizations, and other payers. Prior to The Center, Ms. Twigg held both clinical and administrative positions at the Calvert Health Medical Center, the University of Maryland Medical System Shore Medical Center at Chestertown, and the Johns Hopkins Home Care Group. Ms. Twigg has presented and received recognition for her development of innovative programs targeting avoidable readmission reduction, improving the provision of quality care, and aligning patient advocacy with fiscal accountability. In addition, she is involved in various healthcare-related associations, including MRHA, ACMA, MDDC SHRM, and MDAAP. Ms. Twigg received her Bachelor of Science in Nursing from Salisbury University and a Master of Science in Management from Wilmington University.

 

Anne Conway

Anne Conway is the Director of Health Plan and Hospital Services at The Coordinating Center, a Maryland statewide nonprofit that supports people of all ages and abilities in their aspirations for health, independence, and a meaningful community life. With a wealth of experience in the healthcare industry, Anne excels in overseeing a team of Registered Nurses and Community Health Workers dedicated to providing comprehensive care management and coordination services. Anne's nursing journey spans over two decades, commencing at the HSC Pediatric Center, where she worked as a staff nurse specializing in the care of premature infants and toddlers with chronic medical needs. Additionally, she contributed her expertise as a Clinical Research Coordinator at the University of Maryland Shock Trauma Center in Baltimore. During her tenure there, Anne actively participated in academic research initiatives, investigator-initiated studies, and clinical outcomes research. Prior to joining The Coordinating Center, Anne held the esteemed position of Vice President of Care Management at Health Services with Children with Special Needs, where she made significant contributions to the field. Anne's educational background includes an associate degree in nursing from Anne Arundel County Community College, as well as a BSN and an MS in Clinical Research Management from the University of Maryland School of Nursing.

 

Quiana Craig, cpb-I, cpb, cpc, cdbs

Quiana Craig is a New York-based Certified Professional Biller (CPB) and licensed AAPC Approved Instructor. With over a decade of experience in medical billing, she established her own home-based business in 2008, providing her with the freedom, flexibility, and financial success she desired. Quiana's expertise in managing the revenue cycle and her passion for teaching led her to create the renowned Biller BOSS Bootcamp® course. Through this program, she empowers aspiring professionals to master the skills needed for a successful career in medical billing, including navigating the revenue cycle, utilizing billing software, and establishing a profitable home-based business.

 

Kayleigh hope, MPH

Kayleigh is an Analyst at the Maryland Primary Care Program (MDPCP) within the Maryland Department of Health. In this role, she provides key support for the evaluation of various MDPCP initiatives to advance primary care and inform policy. Kayleigh received her Master of Public Health and certificate in Health Policy and Practice from the Columbia University Mailman School of Public Health. She has a background in Epidemiology and is passionate about health equity and improving health outcomes in vulnerable populations.  

 

Jasmine Leonard, MPH

As CareFirst BlueCross BlueShield’s Health Equity Enterprise Manager, Jasmine is responsible for being the resident good troublemaker, leading all department health equity initiatives, and managing the CareFirst relationship with Children’s National Hospital. With a passion for health equity and over ten years’ experience, Jasmine has paved her way as an advocate for historically excluded, pediatric, and adolescent communities. Since 2020, she has brought thought provoking educational seminars to thousands building knowledge on the historical impacts of slavery, institutional discrimination, and medical mistreatment based on race. Jasmine earned a bachelor’s degree in Human Biology and Africana Studies from Brown University, a Master of Public Health in Public Health Informatics with a Health Equity Focus from the University of Minnesota, and is currently pursuing a Doctorate of Public Health concentrated in Health Equity and Social Justice at the Johns Hopkins University Bloomberg School of Public Health as an Adolescent Health Bloomberg Fellow.

 

Maya Deane-Polyak, MPH

Maya is a Pediatric Population Health Strategy Consultant for the nation’s largest Patient-Centered Medical Home Program at CareFirst BlueCross BlueShield. In her role at CareFirst, Maya partners with 150 pediatric primary care practices and supports their efforts to collect Social Determinants of Health (SDOH) data and implement data-driven solutions that advance health equity and improve quality of care for patients across Maryland. Maya earned a bachelor’s degree in Community Health and a Master of Public Health degree with a concentration in Health Equity, both from the University of Maryland College Park.

 

Marc Rabner, MD, MPH

Dr. Rabner is a highly experienced pediatrician and public health professional. He currently serves as the Chief Medical Officer at CRISP, where he focuses on advancing healthcare delivery systems and improving patient outcomes. With a background in pediatrics and public health, Dr. Rabner has a deep understanding of the complex factors that contribute to health disparities. He is dedicated to addressing these root causes and driving positive change in healthcare through the use of interoperability and data integration.

 

Quinton Askew

Quinton Askew is the President/CEO for Maryland Information Network, 211 Maryland. He was previously a Director with Howard County Government, Department of Community Resources & Services, focused on Community Engagement, Systems Development, and oversight of fifteen nonprofit entities co-located to provide health/human services. In addition, he is a National Consultant for Aha Process, focusing on Enabling individuals, institutions, and communities to stabilize and grow resources for all, particularly those in poverty. 

 

Kimberly Gardner, RD, LD

Kimberly Gardner is an alumna of the Coordinated Program in Dietetics at Howard University. As a practicing registered & licensed dietitian in Indiana, Kimberly utilizes her skills in motivational interviewing in the hospital outpatient setting. Dietitians at her hospital receive referrals from primary care physicians for prediabetes, diabetes, weight loss, and general nutrition education. She utilizes translation services provided by her facility to enhance communication with patients from different cultural backgrounds. 

 

 Kelsey Toups

Kelsey is currently in graduate school, pursuing her Master of Public Health at Boston University. She currently serves as a Learning Intern at the Maryland Primary Care Program Management Office, where she assists with many projects that support the MDPCP Learning System. She has worked with the Quality and Learning Manager and JIA Staffing team to support the delivery of the Staff Training Academy. Kelsey will serve as a co-host on the day of the event— say hi!
