Prescriptions

If you are unable to bring your prescription in to our pharmacy, you can also have a new prescription filled at MY Pharmacy using one of the three methods described below:

1. Mail: You can mail the original prescription to us at:

2. Phone: Ask your doctor to phone the prescription to our pharmacists at MY Pharmacy: 1-123-123-1234

3. Fax: Ask your doctor to fax the prescription at MY Pharmacy: 1-123-123-1234

Note:

• Be sure to send us your full name, date of birth, any known allergies/medical conditions, mailing address, contact number, and method of payment including any third party insurance information that may pertain to your prescription.

• We can only accept prescriptions written by a physician, dentist, veterinarian, nurse practitioner, midwife, or podiatrist licensed in Canada, of whom you are a patient.

• Re-ordering your prescription through our web site is easy and hassle free. Simply complete the e-mail form below. (If you have any trouble using our on-line form, please call us instead.) All fields are required.

• Auto Refills

• Let us take care of refills for you.

• AutoFill is a unique prescription refilling program designed to automatically refill your medications. It's all about your convenience - no more missed doses, less waiting time, and fewer trips to the pharmacy. Your Pharmacist will even notify you when your prescriptions are ready for pick up and when you need to contact your physician for renewals.
