• Please complete a new form for EACH person attending the banquet so we have an accurate count of attendees. 

• KTOY Nominees must register, but do not have to pay. They may designate ONE complimentary guest who must also be registered with this form.

• Deadline for registration is Friday, April 10.

 Please email ktoyregion2@gmail.com if you require an invoice, indicating the names of attendees (each one should first be registered above) for which you are paying, as well as person responsible for payment.
