Higgins Chiropractic in Hutchinson will once again be providing FREE SPORTS PHYSICALS for RVMS and NHS students Monday, August 4th through Friday, August 8th.  

You will need to call Higgins Chiropractic at 620-662-9272 to schedule an appointment for your physical examination.  

Please have your end of the Sports Physical paperwork filled out before your appointment.  Physical packets are available for pickup at the RVMS and NHS front offices.  You can also print them out on our website or click the links below to download all forms.

IMMUNIZATION RECORD: Must be up-to-date, is required for all students.
(If exempt, new paperwork is required for each new school year.)

MEDICATIONS: If your student(s) will take medication or receive any medical treatment at school, please update ALL online Health forms in the online enrollment process.  You can also update your medical authorizations, which permit us to give meds like ibuprofen, Tylenol, etc., while student(s) are in school.
