Please ensure your student has a Physical before school begins on August 13th. Physical packets are available for pickup at the RVMS and NHS front offices.  You can also print them from our website or click the links on the left to download all forms.

IMMUNIZATION RECORD: Must be up-to-date, is required for all students.
(If exempt, new paperwork is required for each new school year.)

MEDICATIONS: If your student(s) will take medication or receive any medical treatment at school, please update ALL online Health forms in the online enrollment process.  You can also update your medical authorizations, which permit us to give meds like ibuprofen, Tylenol, etc., while student(s) are in school.
