Complete the following form to register for the 2023 Clearwater Youth Camp for Boys and Girls 2nd-8th Grade or CHS Boys Basketball Camp for Boys 9th-12th Grade May 30-June 2 at Clearwater High School.  Payment for camp can be made through Venmo (@Dustin-Clevenger-4), PayPal (@dclevenger55) or Check/Cash on the first day of camp.  Please register multiple campers on different forms.  Contact Dustin Clevenger with any questions at dclevenger@usd264.org.  We look forward to getting back in the gym this summer!!
