Evaluation Procedure

For Previously Evaluated:

You may use the filled-up evaluated form you sent during the previous evaluation but complete and encode your new grades from your subjects taken. Be sure to have your Certificate of Rating (COR - your list of subjects with final grades) from the Student Portal, have a softcopy of the image or screenshot. After encoding, save your file. The format of is the same as below:

Then go to the Google Form (Submission Bin) and submit the accomplished Prospectus and the Certificate of Rating (COR - your list of subjects with final grades) (the image or screenshot copy) for verification. From time to time, check your Student Portal if your account is activated and may now proceed to the enrollment.

For New Students not yet undergoing Evaluation of Grades:

1. Download your prospectus. (Link is provided below)

2. Fill it up with your Name (Surname, First Name Middle Initial - Example: RIZAL, JOSE P.) and Student Number. Prospectus 2018 is a fill-up form, while Prospectus 2010 must be printed first, then filled up manually and be scanned.

3. Be sure to have your Certificate of Rating (COR - your list of subjects with final grades) from the Student Portal, have a softcopy of the image or screenshot.

4. Encode your corresponding grade for every subject in the rectangular box provided. An only passing grade of 3.00 & above and INC (Incomplete) will be encoded.

5. After encoding your grade, save your file.
