Our School Nurse, in collaboration with family, staff and community involvement, strive to maximize student learning and to create a healthy school environment. Here at Upper Pittsgrove School, we believe that healthy children learn best. It is our goal to facilitate optimal health, growth, and learning in order for our students to have success in school.
Medical treatment will be administered to your child following Standing Orders from our school physician in accordance with school board policies and best nursing practice. The school nurse will conduct health screenings annually as required by the New Jersey Department of Education. Screenings include checking a student's height, weight, blood pressure, vision, hearing, and scoliosis. These screenings are performed according to the mandated schedule.
Upper Pittsgrove School requires that all newly enrolled students provide a physical examination form upon entry to the school district as per the NJ state guidelines. The physical examination form should be completed by your family healthcare provider. The physical examination is required for any newly enrolled student and must have been conducted within the last 365 days. The State of New Jersey also has immunization requirements that are required for school attendance in accordance with N.J.A.C. 8:57-4.1 et seq for preschool, kindergarten, and sixth grade. Physical exam and proof of immunizations must be submitted to school upon registration for newly enrolled students (must be completed and returned to school within 30 days of registration). It is important to receive medical care for your child at least once during each developmental stage: at early childhood (preschool- grade three) , pre-adolescence (grades four through six) and adolescence (grades seven - twelve).
