PLAN OF STUDY (POS)
GE POS
1. Download the GE POS Form.
2. Consult with your adviser.
3. Fill out the form properly; *filename: SURNAME - GE POS
4. Have the form signed by your adviser. (Wet signature)
5. Submit the form to OCS
6. POS will be emailed to student, cc; Adviser
Technical & Social Electives
1. Request the BSDC POS Electives Form / ASDC POS Elective Form
2. Consult with your adviser.
3. Fill out the form properly; *filename: SURNAME -POS
4. Have the form signed by your adviser. (Wet signature)
5. Submit the form to OCS
6. POS will be emailed to student, cc; Adviser
POS Revision (Before or During Registration only)
1. After consultation with your adviser, fill out the POS Revision Form. Request Form
[Attachments: Approved POS, Timetable, POS Rev Form, and or Medical Certificate, if any (for health reasons)]
2. You may pick up the POS Revision Form at the OCS, Rm. 103B
3. Fill out the form properly.
4. Have the form signed by your adviser. (Wet signature)
5. Submit the form to OCS
6. Approved POS Revision will be emailed to student, cc; Adviser
Note:
1. Your revision will ONLY be approved if it satisfies any of the following conditions:
a. Health reason (present a medical certificate)
b. Subject is seasonal, and student is graduating; or
c. non-satisfaction of prerequisite and student is graduating.
d. Other justifiable reason/s. (case-to-case basis)
2. One email thread should be used for the whole process.

Consent of Instructor (COI)
Download COI Form (via AMIS)
Fill out the form and have it signed by the faculty-in-charge and the head/department chair of the unit offering the course.
For Devcom courses, upload the accomplished form: COI-DEVC
For non-Devcom courses, email to the Registration Committee/OCS of the Unit offering the course and upload a copy for CDC-OCS: COI-Non-DEVC
Enlist the course after receiving the notification via email (re: SAIS tagging).
Email or Visit OCS for instruction regarding Cross-Register
Submit a letter of intent (attached form 5) addressed to the College Secretary and noted by your Adviser and parent legal guardian at OCS. (wet signature/s is required)
Once approved, OCS will email you the payment instruction. After payment you may pick up your dropping form.
Fill out the form properly and have it signed by your adviser and your faculty-in-charge.
Submit the completed form at the OCS for College Secretary’s signature.
Once signed, submit the approved copy of the dropping form to your FIC.
NOTE: Deadline of filing Dropping for 2nd Semester AY 2024-2025: 25 April 2025
(With accomplished Dropping Form and Dropping Fee)
Submit a letter of intent (addressed to the Dean), signed by the adviser and parent/legal guardian (wet signatures are required) to OCS.
NOTE: Deadline of filing Leave of Absence for 1st Semester, AY 2025-2026: 19 November 2025
(With accomplished LOA Form, College Clearance and LOA Fee)
Request a Excuse Slip form at OCS Rm. 103B
Bring a valid proof of reason for attachment (Medical Certificate from UHS, if Medical certificate is from private physician, submit it to UHS for validation)
Once approve by OCS, have the form signed by your FIC then return the slip to the OCS
Note: For death of immediate family member, death certificate is also valid as proof of reason for Excuse Slip
Request for Removal of 4.0/Completion Permit (Request Permit).
You will received an email to pick up your permit.
Fill out the permit and have it signed by your FIC. (Wet signature)
Return the signed removal permit to OCS.
The approved removal permit with the completion grade sheet will be forwarded to your FIC.
Fill out the request form.
Follow payment instructions sent via email.
College Clearance
Fill out the request form.
Note: Students who are eligible for the free tuition at the time of graduation are not required to pay the graduation fee. For those required to pay, instructions will be sent via email.

Double check your display information as it is vital your details are correct and true for your clearance
Choose the purpose of the clearance in order to proceed then click apply.
Note: Don't have an OSAM account? Visit OSAM-Account-Reg and fill out the necessary information. Wait for an email confirmation of your OSAM Account creation within 2-3 business days.

Accomplish the form and have the form signed by your adviser.
Upload your application form. Upload-Grad-Application.
Upload a copy of the approved POS and revision of POS (if any).
Submit your Letter of Intent and updated Timetable signed by your adviser at OCS. (Wet signatures are required)
Please be reminded of the following valid reasons for granting an underload certificate:
1. Health concerns (a medical certificate is necessary)
2. Employment reasons (a copy of the employment appointment and payroll must be submitted)
3. Unavailability of course/s - The remaining course/s is not currently offered - the course is seasonal or abolished (OCS will inquire with the Department concerned.)

NOTE: Overload Permit is only allowed for senior/graduating students.
