Guide on how to borrow and renew a book online
Fill out the online form to proceed with your request.
You will receive an email about the availability of the book for pick up/receipt of the renewed book/s.
Pick up the book/s at the designated area of the unit library.
Reminders:
Use this form or scan the QR code provided to borrow/renew a book from the University of the Philippines Manila Libraries.
We strongly suggest that you consult the Tuklas Web Catalog to search for book/s to be loaned out. Take note of the following: call number, author, title, and location of the book/s to determine the exact location of the book/s in the library to be visited.
ONLY Bona fide users are allowed to borrow using the library online form.
Only books from Reserve, Filipiniana, and General Circulation Collections can be borrowed.
When submitting your request, please ensure that the status of the book is "On Shelf" which means that the item is available in the library.
In case you have requested a book that is currently "On Loan", we will put your name on the queue and the unit library will notify you once the book becomes available.
The cut-off for receiving requests is until 4:00 PM. Any requests beyond this period will be processed on the next business day.
You will be notified via email when the book is ready for pick-up, or when the book you returned has accrued late fines.
Books must be picked up or dropped off at the designated area of UPM libraries.
Borrowing/renewal is not allowed if there is a pending book due for return
Borrowing Privileges
UP Manila constituents can borrow materials from any of the libraries in the system. The number of items you can borrow depends on your status and the loan category of the item. A range of categories is used to facilitate the most effective use of our collections. You must have your University ID card with you to borrow items from the library. If an item has been reserved for you, it will be held at the Issue Desk. Items can be returned at the Issue Desk.
Borrower's Responsibilities
You are responsible for ensuring that items are returned or renewed on or before the due date.
Please note that there is no guarantee that it will be possible to renew items. If items have been reserved for another user, they must be returned on the due date. Fines will apply if items are not returned by the due date.
Any borrowed item may be recalled at any time. Fines will apply if items are not returned by the amended due date.
Please remember that you are responsible for all items borrowed from your account until they are returned and cleared off your record.
Overdue Materials
If items become overdue while issued to you we will send a courtesy reminder by email or post informing you of this.
If you fail to return them at this stage a second overdue message will be sent and your record will be blocked for borrowing and renewal.
If you still fail to return items you will be billed for their replacement cost.
Notices are a courtesy and non-receipt does not exempt the borrower from overdue fines or charges.
Due to the restricted number of accounts assigned to UPM, only faculty and staff have user accounts available on the Turnitin database. If your manuscript / research paper needs to be evaluated, please see the instructions below:

Scan and Deliver Service is a free electronic document delivery service for the University of the Philippines Manila community. In light of the new normal, we are offering this service to help library users who can't go to the library to borrow resources.

How to avail of this service?
To avail of this service follow the steps below:
Visit UP Tuklas Web Catalog to search for print books and journals available in UPM libraries
Kindly fill up the request form or email your request directly to the Medical Library at fbhmedlib.upm@up.edu.ph with complete details.
Please send all pertinent information concerning the materials you've requested : For book chapter request | Call Number, Book Title, Author, Chapter Title; For article request | Journal Title, Volume/Issue No., Article title, DOI (if available).
If available, the requested material will be sent to your email.
Terms of Service
Limitations on Scan and Deliver Service are related to the capacity of available staff resources.
Requests are limited to one journal article/book chapter per title only or not exceeding ten percent (10%) of the work.
Requests for more than one hundred pages or encompassing entire works is not allowed.
Scan and Deliver Service reserves the right to process one request per person per day, depending on staffing and over-all request volume.
Some collections may not be eligible for Scan and Deliver Service.
If requests are otherwise not in the spirit of terms of service, the College Librarian will contact you by email.
Document Delivery Service is an electronic document delivery service for the UP Manila, UP System, and Consortium members/constituents. This service delivers an online journal article or book chapter to you via email for your research and academic needs.

The service enables library users to request the purchase of a specific book, journal, or database as part of the library collection or subscription. Acquisition of recommended items is subject to approval and funding availability. However, we make every effort to honor all recommendations made by our valued library use

For CM Undergraduate Students
Application for college clearance is through the UPCM web portal. Log in using your UP email and click on the college forms.
If without accountability, your application will be verified by the College Librarian. Otherwise, an email notification will be sent to your official UP email (@up.edu.ph) with complete information about your library accountability.
Verified clearance can be generated in the portal after completing the three (3) stage process.
For CM Graduate Students (MS and PhD Program)
Submission of thesis/dissertation for graduating students is a requirement for library clearance.
1. The thesis should be its final and approved version.
2. The following options are available for submitting a library copy:
2.1. Hardbound copy
It can be submitted by the author or an authorized representative to the library.
2.2. Electronic copy in PDF format via email
You can send the copy to: fbhmedlib.upm@up.edu.ph; cc: thesis adviser, and the University Library.
PDF file name format: LAST NAME, FIRST NAME and MIDDLE INITIAL

3. There is no deadline for submitting the thesis/dissertation. However, in order to avoid any inconvenience, please ensure that you have accomplished this requirement before requesting library clearance.
4. Secure a printed College clearance form from your respective department. Have the printed copy of clearance signed on-site or scan and send it to fbhmedlib.upm@up.edu.ph
For PGH Employees / Residents / Fellows
Secure an electronic copy of the Clearance Form from the HRDD or scan the print copy of the form.
Using your official UP email (@up.edu.ph), send the form to Ms. Carla G. Demabildo, College Librarian-In-Charge of the College of Medicine (F.B. Herrera, Jr. Medical Library) via fbhmedlib.upm@up.edu.ph.
If without accountability, your clearance will be sent back to you signed by the University Librarian. Otherwise, the form will be sent back unsigned with complete information about the unreturned book/s or the amount of fines. It is advised to settle your accountability first before having your clearance signed.
This form is intended for UPM Faculty, REPS, Admin, and Students requesting accounts for Labster. Please note that the student account is limited to 1,000 slots only.
