Fill-out this form complete with signature and save as pdf or jpg. This will serve as an attachment to one of the mandatory items in the google form.
2. Depending on the type of membership, kindly prepare the necessary documents for uploading,
a. For Associate membership, kindly upload your recent registration form/ Valid School ID
b. For Life Membership, kindly upload membership certificate and at least 2 certificates of participation in ASNP-PH activites
3. Identify the membership type, make sure that you have the supporting documents as mentioned above and proceed with online payment. For online bank payment, kindly use the following details:

3. Once you've prepared your Membership Form, Proof of Payment, and Supporting Documents, proceed by uploading files using the Google Form. Upon submission, you will receive a confirmation email verifying receipt of your application.
Interested in being a member? For those residing in Thailand and Korea, membership application should be submitted to the secretary of the ASNP National Chapter. For those from other countries, the application form should be submitted to the ASNP main office, asnpmail@gmail.com.
