Sport Donation:
How to Register
Step 1: Please click through and read ALL of the Athletic Forms below ⬇️ before filling out the Google Athletics form below ⬇️ .
Step 2: Students MUST complete the Athletic Form registration process before trying out for or joining a DMS team. ***You MUST fill out the Athletic Form before tryouts begin in order to be eligible for tryouts.***
(Please check the individual Sports page for the tryout, practice, and game schedules to ensure there are no scheduling conflicts!)
Step 3: Once you make the team, please donate to the DMS Athletics Program.

The Athletic Form will be open in August 18th,2025. Please look back then.
Complete the Athletic Form for your student-athlete. This form only needs to be complete once per athlete.

Note to DMS Parents:
Athletics are funded solely by donations made by parents/guardians and families. Donations are suggested if your child makes a DMS team or joins a non-cut DMS team. Please help support Dartmouth Athletics by making a $150.00 donation per team your child participates in to support our sports program.
Donations can be made by cash or checks. (Please make checks payable to Dartmouth Middle School.)

Company Match Program:
Select "Union School District" (in some systems it is "Union Elementary School District")
In the comments type "[Athlete's Name] Dartmouth Athletic [sport team] Program"
This will ensure your Donation will make it to us!
