The purpose of this project was to better understand the health needs of and systems gaps in health services for youth experiencing homelessness in and around Hennepin County.

This project aimed to address the health-related objectives identified in Hennepin County's Youth and Young Adult Coordinated Community Plan to effectively prevent and end youth homelessness. This plan was developed in collaboration with youth, youth-serving agencies and other partners across sectors, as part of Hennepin County's Youth Homelessness Demonstration Project.

The project leveraged our existing collaboration with community and cross-sector partners to focus on improving health and health services for youth experiencing homelessness in and around Hennepin County. The findings served as a base for our continued Systems Alignment to Optimize Health Services for Youth Experiencing Homelessness work.
