We understand that critical work must continue during this downtime. See information below regarding accommodations made for submitting Reportable New Information and Continuing Review information to avoid lapse of IRB approval.
Submitting Reportable New Information (RNIs)
To submit Reportable New Information (RNI) related to study suspension, study termination, an unexpected participant death, or information related to the safety, health, or well-being of participants or others during ETHOS downtime, please do so using this Qualtrics form. RNIs not related to suspensions, terminations, an unexpected participant death or the safety, health, or well-being of participants or others should be submitted via ETHOS once it is back online, unless the PI determines more urgent IRB notification/review is warranted.
Submitting Requests for Continuation of Subjects should the Study Lapse during the Upgrade
Continuing human research procedures after approval has expired is a violation of university policy and IRB requirements.
If the study lapses, human research procedures related to the protocol under review must cease, including recruitment, advertisement, screening, enrollment, consent, interventions, interactions, and collection or analysis of private identifiable information.
If current subjects will be harmed by stopping human research procedures that are not available outside the human research context, immediately submit this form and request IRB approval to continue specific activities. Please refer to HRP-063 for additional information regarding what the IRB reviewer will consider.
Investigators may be required to submit a Report of New Information (RNI) to report this as non-compliance after the ETHOS Upgrade.
Grant Administrator and Certified Approver Access to Funding Reports
A copy of the funding report is available to Grant Administrators and Certified Approvers via UMN Box Secure Storage. You must request access to the Box folder by completing this form. All UMN employees have a Box account but it does require activation. If you have not done so previously, please follow the instructions on this job aid to request activation of your Box account. You must activate your Box account before requesting access to the folder.
