The Use and Effect of Community-Based Structures on Inequality

Paper:  Mixed-Methods Evaluation of the Bi-Directional Impact of the HealthPartners Community Advisory Council for Research and Evaluation

Abstract: Community Advisory Councils (CACs) are increasingly recognized as key partners in advancing health equity. Yet there is limited rigorous evaluation of their bi‑directional impact. To address this, the HealthPartners CAC for Research and Evaluation used a bi‑directional impact framework to assess how council recommendations influence organizational decisions and how institutional responsiveness affects council engagement. Methods: A mixed‑methods evaluation combined qualitative document review with stakeholder‑engaged data collection. Key council documents were analyzed to understand the CAC’s purpose and context. A structured participatory session with CAC members and HealthPartners staff gathered insights on the council’s contributions and the organization’s responsiveness. All qualitative data underwent rapid thematic analysis to identify recurring themes across both measurable outcomes and relationship‑based impacts. Results: Eight core themes emerged: Trust & Transparency; Community Engagement & Partnership Building; Equity & Culturally Relevant Practices; Social Drivers of Health; Resource Allocation; Impact Measurement & Improvement; Enhanced Health Care & Plan Delivery; and Opportunities for Growth. The evaluation highlighted increased organizational responsiveness, reflected in changes to research design, outreach, health care delivery, and public health priorities. CAC members reported strong engagement and alignment with community needs. Conclusions: Effective impact tracking requires attention to both relationships and metrics. Findings show that intentional, bi‑directional evaluation strengthens trust, accountability, and commitment to equity, reinforcing the CAC’s value in shaping institutional practices.
