System Notes:
This is a new reservation form (2022), so you may need us to reactivate your previous user ID
Request form is used for in-person, virtual, or hybrid events
Requests can be made no more than 1 year in advance
All reservations must contain PFA (pre-function area, the lobby inside building)
Please include as much information as possible in the request

After your request has been approved from QI, submit your event to the Events and Activities Intake Form

Click "book now" with the request for event template.
