After each session, you are asked to complete a survey and provide feedback. Use the form on this page, or click here, every time you want to finish a survey. A copy of your responses will be sent to the email you provide in the survey.

Reminder

Trainees are required to complete ALL surveys in order to receive the Certificate of Completion. We recommend you complete your surveys immediately following each session or no later than 3 days to ensure you do not fall behind. Please see the full policy below for details.
