Submit the following via email to youthprotection@uah.edu & cc athleteaffairs@uah.edu These forms can all be found by navigating to the website link above.

• Program registration form

• Staff information form

• Child abuse or neglect report form

• Proof of Insurance Parent Package template 

• Background checks or complete (in full) the background attestation form (this requires a notary)

• Camp preauthorization form

Complete and submit the new facilities use agreement  

• 1) Complete the form and send to Tonya Welker and Courtney Adcock

• 2) Either Tonya or Courtney will return to you. 

• 3) Submit the completed form (signed by Dr. Smith and Dr. Karr) to youth protection and athlete affairs
