Parents/Guardians,

Here is the link where you can find a printable copy of the 2020/2021 Choices After School Program registration form. Please visit www.tcsdk8.org and look for CHOICES to find our Application for Registration. Once you find our Registration Form, you will be able to fill it out, print, and sign/initial in the appropriate spaces before you turn in your form to Sunnyside's office. Thank you!
