Welcome to register for the "1st Asia Pacific Hospital at Home Congress 2026 (APHaH 2026)" (hereinafter referred to as "the Congress"). To protect your rights regarding participation and to comply with the Personal Data Protection Act of the Republic of China (Taiwan), please read the following terms and conditions carefully before registering. By checking "I Agree" or completing the registration and payment process, you acknowledge that you have fully understood and agree to abide by all the regulations set forth below.
Part I: Registration & Cancellation Policy
1. Important Dates To ensure your rights regarding participation, please pay close attention to the following deadlines (all times are in Taiwan Standard Time, GMT+8, 23:59): (1) Early-Bird Registration: February 10, 2026, to April 30, 2026 (2) Author Registration Deadline (Required): April 30, 2026 (3) General Registration: May 1, 2026, to June 10, 2026 (4) Visa Letter Request Deadline: May 31, 2026 (5) Conference Dates: June 27–28, 2026 (Note: All registration fees must be paid in full by the corresponding deadlines. Failure to complete payment within the designated timeframe will result in the system automatically adjusting the fee to the next tier's rate, or the cancellation of paper presentation eligibility.)
2. Cancellation & Refund Policy If you are unable to attend the Congress for any reason, you must notify the Congress Secretariat in writing via email to process your cancellation and refund. The refund amount will be calculated based on the date the written cancellation notice is received by the Congress: (1) More than 30 days before the event (on or before May 27, 2026): A 90% refund of the registration fee will be issued (the Congress will deduct 10% for administrative processing fees). (2) 14 to 30 days before the event (between May 28 and June 13, 2026, inclusive): A 50% refund of the registration fee will be issued. (3) Within 14 days of the event (from June 14, 2026, until the end of the Congress): No refunds will be granted. (Refund Guidelines: No-shows who fail to submit a written cancellation request within the specified deadlines will not receive a refund. All approved refunds will be processed within one month after the conclusion of the Congress. Any bank fees or exchange rate differences incurred from cross-border wire transfers or credit card chargebacks shall be borne by the attendee.)
3. Substitution Policy If you are unable to attend after completing your payment, you may submit a written request to the Congress to transfer your registration to another individual from the same organization before June 10, 2026 (the general registration deadline). The Congress will not charge an additional processing fee for this transfer. Substitution requests received after this date will not be accepted.
4. Force Majeure & Program Changes In the event that the Congress must be postponed, downsized, or canceled due to force majeure factors such as natural disasters (e.g., typhoons, earthquakes), pandemics, strikes, or wars, the Congress will notify attendees as soon as possible. Under such circumstances, the Congress shall not be held liable for any transportation, accommodation, or other personal losses incurred by the attendees as a result of their participation. The Congress reserves the right to refund the remaining registration fees on a pro-rata basis after deducting any preparatory costs already incurred.
Part II: Privacy Policy & Consent Form
1. Data Collection Agency and Purpose The purpose of collecting your personal data by the Congress Organizer (Taiwan Society of Home Health Care) is to process APHaH 2026 Congress registrations, verify identities, collect payments, send pre-event notifications, communicate Congress-related information, issue certificates/credits, conduct satisfaction surveys, and for subsequent academic exchange and conference statistical analysis.
2. Categories of Personal Data Collected The data collected includes, but is not limited to: name, gender, affiliated organization, job title, contact phone number, email address, mailing address, ID card number/passport number (exclusively for continuing education credit applications or specific administrative review purposes), and dietary preferences (to facilitate Congress catering arrangements).
3. Period, Territory, Targets, and Methods of Personal Data Utilization (1) Period: From the Congress preparation phase until three years after the conclusion of the event, or within the retention period mandated by law. (2) Territory: The operating regions of the Organizer and relevant outsourced partners (such as payment gateway and registration system providers), as well as regions where Congress-related operations are executed. (3) Targets: The Organizer, co-organizers, authorized third-party service providers handling payments and system maintenance, and relevant societies issuing academic credits. (4) Methods: Collection, processing, and utilization via automated machines or other non-automated means.
4. Exercise of Data Subject Rights and Consequences of Non-Provision In accordance with Article 3 of the Personal Data Protection Act of the Republic of China (Taiwan), you may exercise the following rights regarding your personal data held by the Organizer: inquire, request to review, request to make copies, supplement or correct, demand the cessation of collection, processing, or utilization, or request deletion. You are free to choose whether to provide your personal data; however, if you refuse to provide the necessary information required for registration, the Organizer will be unable to complete your registration review or provide Congress-related services such as credit certification.
5. Portrait Rights and Audio-Visual Release The Organizer will conduct video and photo recording during the Congress. By registering for the Congress, you consent to the Organizer's legal, non-profit use of photographs or videos containing your likeness for Congress outcome reports, the official website, social media promotion, or educational outreach.

歡迎報名參加「2026 第一屆亞太在宅醫療會議 (APHaH 2026)」(以下簡稱本大會)。為保障您的與會權益並遵守中華民國《個人資料保護法》之規定,請於註冊前詳細閱讀以下條款。當您勾選「我同意」或完成報名繳費程序,即表示您已充分瞭解並同意遵守以下所有規範。
第一部分:大會註冊與退費條款 (Registration & Cancellation Policy)
一、 重要報名時程:
為確保您的與會權益,請務必留意以下各階段之截止日期(皆為台灣標準時間 GMT+8,23:59 截止):
(一) 早鳥報名期間: 2026 年 2 月 10 日至 2026 年 4 月 30 日
(二) 論文作者註冊截止日(必要): 2026 年 4 月 30日
(三) 一般報名期間: 2026 年 5 月 1 日至 2026 年 6 月 10 日
(四) 簽證邀請函 (Visa Letter) 申請截止日: 2026 年 5 月 31 日
(五) 會議舉辦日期: 2026 年 6 月 27 日至 28 日
(註:所有註冊費須於上述對應之截止日期前完成全額繳納。未於期限內完成繳費者,系統將自動調整為下一階段之收費標準,或取消其論文發表資格。)
二、 取消報名與退費規定
若您因故無法出席會議,必須以書面電子郵件通知大會秘書處辦理取消與退費。退款金額將依據大會收到您的書面取消通知日期進行計算:
(一) 活動前 30 天以上(2026 年 5 月 27 日(含)前): 退還 90% 之註冊費(大會將扣除 10% 作為行政處理手續費)。
(二) 活動前 14 至 30 天(2026 年 5 月 28 日至 6 月 13 日(含)期間): 退還 50% 之註冊費。
(三) 活動前 14 天內(2026 年 6 月 14 日(含)起至會議結束): 恕不予退費。 (退費須知:未出席會議且未於期限內提出書面申請者恕不退費。核准之退款將於大會結束後一個月內辦理,跨國匯款或信用卡退刷產生之手續費或匯差由與會者自行負擔。)
三、 報名資格轉讓:
若您完成繳費後因故無法出席,可於 2026 年 6 月 10 日(一般報名截止日)前,以書面形式向大會申請將報名資格轉讓給同單位的其他人員,本會不另收手續費。逾期恕不受理轉讓。
四、 不可抗力因素與大會變更:
若因天災(如颱風、地震)、疫情、罷工、戰爭等不可抗力因素,導致會議必須延期、縮減規模或取消,大會將盡速通知與會者。在此情況下,大會不負擔與會者因參與會議所衍生之交通、住宿或其他個人損失。大會保留扣除已發生之籌備成本後,按比例退還剩餘報名費之權利。
第二部分:個人資料蒐集、處理及利用同意書 (Privacy Policy & Consent Form)
一、 蒐集機關名稱與目的:
本大會主辦單位(社團法人台灣在宅醫療學會)蒐集您個人資料之目的,係為辦理 APHaH 2026 大會之報名註冊、身分確認、費用收繳、寄送行前通知、大會相關訊息聯繫、證書/積分發放、滿意度調查,以及後續之學術交流與會務統計分析使用。
二、 蒐集之個人資料類別:
包含但不限於:姓名、性別、服務單位、職稱、聯絡電話、電子郵件信箱、通訊地址、身分證字號/護照號碼(僅限申請繼續教育積分或特定行政審查使用),以及飲食習慣(以利安排大會餐飲)。
三、 個人資料利用之期間、地區、對象及方式:
(一) 期間: 本大會活動籌備至活動結束後三年內,或依法規必須保存之期限。
(二) 地區: 主辦單位、相關委外合作夥伴(如金流與報名系統商)所在之營業地區,及本大會業務執行地區。
(三) 對象: 主辦單位、協辦單位、授權辦理金流及系統維護之第三方服務供應商,以及核發學分之相關學會。
(四) 方式: 以自動化機器或其他非自動化之方式進行蒐集、處理及利用。
三、 當事人權利之行使與不提供之影響:
依據中華民國法律《個人資料保護法》第 3 條,您可向主辦單位查詢、請求閱覽、製給複製本、補充或更正、停止蒐集處理利用,或請求刪除您的個人資料。您可自由選擇是否提供相關個人資料,惟若您拒絕提供報名所需之必要資訊,主辦單位將無法為您完成報名審核或提供學分認證等大會相關服務。
四、 肖像權與影音授權:
本大會活動期間,主辦單位將進行錄影、拍攝等影像紀錄。報名本大會即表示您同意主辦單位將含有您肖像之照片或影片,作為大會成果報告、官方網站、社群媒體宣傳或教育推廣之合法非營利使用。
