Magnus Health Information

Trinity School student medical information is managed through the Magnus Health Student Medical Record (SMR) Portal. Please sign in to the Veracross Parent Portal and click on the Magnus Health link to find medical forms and to upload or update your student's health and medical  information.

Accessing Your Account

To access your Magnus Health SMR account:

Visit the Veracross Parent Portal and click on the Magnus Health SMR Portal button. You may also access Magnus from the Parent page of the website by clicking the Magnus Health Portal link there.

To access your Magnus Health mobile app:

Log into Magnus Health via the Veracross Parent Portal, and look in the top left corner for the little icon that says Mobile App. Click on that link to view instructions for setting up your account within the application. The Magnus Mobile v2 application is available for download in your mobile App Store.

What to Update and Upload

The following information needs to updated and uploaded into Magnus Health by Trinity families. Please note that the mandatory student physical requirements and forms for all students (as indicated below), along with the current information for each child, are in Magnus.

Please do not send this information to Trinity. Trinity faculty and staff cannot upload your family's health information into Magnus. This is for the protection and confidentiality of your information. Rather, please use the tools in Magnus Health to securely submit all health information.

Mandatory for all grades:

•   Magnus Vital Health Record (VHR)

Mandatory for all new students:

•   Enrollment Health Assessment Form

•   Complete Immunization Record

Mandatory for all student-athletes:

•   Athletics Physical Form (updated annually)

•   Student-Parent Concussion Form (as needed)

As applicable:

•   Medication Administration Form

•   Asthma Action Plan

•   Food Allergy Emergency Care Plan

•   Diabetes Medical Management Plan

•   Seizure Action Plan
