Henry J Austin will be visiting PJ Hill Elementary School Wednesday, December 15th, 2021 with their mobile van between 10am - 2pm to provide the following Services: physical exams, immunizations, screening.
Announcement
To Parents/Guardian : _________________________________________________________ The New Jersey law mandates that all students attending school be properly immunized and have a physical on file. According to your child’s school health records, your child is missing either immunizations or a physical that are required to attend school. Your child will not be allowed to return to in-person learning until we obtain proof that your child has received the appropriate immunizations and/or physical.Please provide the school nurse with your child’s up to date immunization and/or physical record as soon as possible. The school nurse has resources that may help if your child does not have health insurance or access to medical care. Contact your school nurse if you need have any questions or need assistance.Attention PJ Hill Parents:
If your child is deficient in immunizations and Physical exam, the NJ state law mandates that all children attending school be properly immunized unless medically or religiously exempt with proof of documentation.
Please make the appointment for your child to receive the required immunizations. Your child must have all his/her immunizations and physical exam on file.
