2. Duly accomplished Personal Data Sheet (PDS) with recent passport-sized picture together with the Work Experience Sheet (CSC Form No. 212    

    Revised 2017) which can be downloaded at www.csc.gov.ph 

3. Authenticated copy of Diploma and Transcript of Records (for new entrants/applicants) 

4. Copy of Certificate of trainings/Seminars attended 

5. Copy of Eligibility (Career Service Sub-Professional/Professional; Board Rating; PRC ID license) 

6. Copy of Performance Rating for the last rating period (for applicant applying for promotion in a regular/permanent position) 

7. Certificate of Employment with statement of duties and responsibilities of previous/current employment
