Forms 101

Google Forms is an excellent way to conduct a survey, or to set up a quiz, exit ticket or a Formative Assessment. When you first create a Form, it is set by default to the survey mode, and will not self grade. You can choose to make it a quiz as you will see in the first video and it will grade itself.

This is a short presentation on how to set up your Google Form as an Exit Ticket/quiz or Formative Assessment.

This presentation explains how to review submissions and responses to your Form, and how to send the form out for your students can take it and fill it out.

This is also easily added to your Google Classroom and can be an effective tool to check for understanding as you move through skills and material.

If you will have multiple groups or periods/sections taking the same Exit Ticket, you will want to add a question that asks what period/section, etc. at the beginning of the Form so that you can sort your results class by class. Make sure that it is not a graded question.
