List below are the Requirements in filling MATERNITY REIMBURSEMENT (MAT2)NORMAL DELIVERY
- MAT 2 Form (can be taken at SSS Branch)- Certified True Copy of Birth Certificate- Medical Certificate - OB History Form (Filled by the attending physician)
- MONTHLY SALARY CREDIT or ACTUAL PREMIUM (can be taken at any SSS branch)
CAESAREAN DELIVERY- Mat 2 Form ( can be taken at any SSS Branch)- Medical Certificate - Certified True Copy of Birth Certificate - OB History Form (Filled by the attending physician)- Operative Technique - Discharge Summary - Maternity Notification (w/ received from the SSS with ultra sound report)
- MONTHLY SALARY CREDIT or ACTUAL PREMIUM (can be taken at any SSS branch)
DELIVERY OTHER THAN THE HOSPITAL (Lying-in/Residence)- MAT 2 Form (can be taken at SSS Branch)- Certified True Copy of Birth Certificate- Medical Certificate - OB History Form (Filled by the attending physician)
- MONTHLY SALARY CREDIT or ACTUAL PREMIUM (can be taken at any SSS branch)
DUE TO MISCARRIAGE - MAT 2 Form- OB History Form- Medical Certificate - Operative Record- Clinical Abstract- Discharge Summary- Histopath Report- Ultrasound Report
- MONTHLY SALARY CREDIT or ACTUAL PREMIUM (can be taken at any SSS branch)
NOTE:
All documents to be submitted must be ORIGINAL/CERTIFIED TRUE COPY together with all the documents are SSS ID/E6 PLUS 2 VALID ID's.
