Magnus Health is a secure website for submitting health information. New parents will receive a Welcome Email from Magnus Health that includes login information; returning parents will receive a notification from Magnus Health asking you to update your child's information. 2025-2026 vehicle decals are available for pick up from the Welcome Center beginning in August, after registering your vehicle(s) using the online form linked above. All student, parent, and employee vehicles must be registered at the Welcome Center or the Risk Management office to receive a TMI Vehicle Decal. You must clearly display it by placing it on the upper right side (passenger side) of the windshield, below the tinted area.
Athletic Forms
As a TAPPS member school, we are required to collect specific athletic participation forms for all student-athletes before they may begin practices or compete. These forms include the Pre-Participation Physical Evaluation and TAPPS Medical History which you can find in the Magnus Health System.
Other required documents that we will need are the Previous Participation Athletic Form and Student Transfer Form. All forms must be completed, signed, and submitted to the athletic department prior to the first day of practice. Failure to submit the required documentation may result in a delay in your child’s ability to participate. Please ensure all forms are turned in on time to help us maintain compliance and ensure the safety of all our athletes.

Through our partnership, your child can be tested and treated for Strep, Flu, and COVID-19 as well as sore throat, headache, skin rash, pink eye, upset stomach, and the common cold. Goodside Health even allows children to receive age-appropriate mental health screenings with parental consent. The medical providers can treat all students regardless of residency or insurance status with most visits delivered at little to no cost to families and translation services are available. Parents can also be included in the telehealth visit without having to leave work!

Throughout the school year, the Advancement Office hosts events for the entire TMI community to participate in and support. Completing this form ensures that your student’s grandparents receive an invitation to Grandparents’ Day at TMI, and other invitations and school news.
