The Travel Boutique LLC is excited to assist you in creating travel adventures and memories that last a lifetime.  It is our goal to exceed your travel expectations during all components of the travel process. The Terms and Conditions written on this webpage shall manage your use of our website, The Travel Boutique LLC accessible at thetravelboutiquellc.com as well as your use / continued use of The Travel Boutique services.  These Terms and Conditions will be applied fully and affect your use of this website and all services provided by The Travel Boutique. By using and continuing to use this website and our services, you agree to accept all Terms and Conditions written here. You must not use this website or services if you disagree with any of these standard Terms and Conditions.

Terms & Conditions

The following Terms & Conditions (the "Agreement") describe what you can expect from us. The Travel Boutique LLC referenced, going forward here, as The Travel Boutique is one and the same company.  The terms "we", "us", "our", and "Travel Agency" refer to The Travel Boutique and the terms  "I", "they", "traveler", "passenger", "group leader", or "you" refers to the individual who has accepted the terms of such agreement.  If you are obtaining a proposal for and/or securing or procuring travel for more than just yourself, all references below that reads “I” (and derivations thereof, including without limitation “you”) shall be read to mean you on behalf of yourself and each individual within your group for whom you are obtaining a proposal and/or securing or procuring travel.

THIS IS A LEGALLY BINDING WORKING, RELEASE, WAIVER OF LIABILITY, AND ASSUMPTION OF RISK AGREEMENT (the “Agreement”). Please read it carefully before signing/accepting/acknowledging/using/continuing to use our website or services.

I understand that The Travel Boutique, is not the source or supplier of the travel services, airline travel, or any third party operated excursion or activity that I have requested, and acts solely as an advisor for the actual suppliers of such services. I have been advised that the suppliers whose names appear in the information supplied to me are those who are actually responsible for providing the services I have purchased. I consent to and request the use of those suppliers and agree not to hold The Travel Boutique responsible should any of these suppliers: 1) fail to provide the travel services I have purchased; 2) fail to comply with any applicable law; or 3) engage in any negligent act or omission that causes me any sort of injury, damage, delay or inconvenience. The Travel Boutique can accept no responsibility for losses or additional expenses due to delay or changes in air, sea or other services, sickness, weather, strike, war, quarantine or other causes.

Force Majeure |  There may be times when either party is unable to perform, or complete performance, under the travel contract for reasons out of each other’s control. These are called force majeure events and if occurs, make performing under the contract inadvisable, commercially impracticable, illegal, or impossible. Events that may trigger this provision include, but are not limited to, acts of God, acts of government, acts of war or civil unrest, insurrection or revolts, military action, strikes or other labor activities, criminal or terrorist activities of any kind, or the threat thereof, pandemics, epidemics, illnesses or health conditions prevalent in the area of travel, earthquakes, hurricanes, lightning, and explosions, unexpected legislation, or any other event outside the reasonable control of either party. In the event that a force majeure event occurs, the parties will look to and follow the cancellation policies of the suppliers, what is stated in the travel proposal and these Terms & Conditions. Any changes to those policies are at the sole discretion of the supplier (if applicable) and The Travel Boutique.  Travelers will be informed of their options if a force majeure event occurs. 

I further agree that neither The Travel Boutique nor its parent, affiliates, subsidiaries or representatives will be or become liable or responsible in any way for any loss, costs, expense, injury, accident or damage to person or property resulting directly or indirectly from (i) the acts or omissions of the suppliers, including, but not limited to, delays or cancellation of services, cessation of operations, breakdown in machinery or equipment, technology problems, or changes in fares, itineraries or schedules, and/or (ii) acts of God, fire, acts of government or other authorities, wars, acts of terrorism, piracy, civil unrest, strikes, riots, thefts, pilferage, pandemics, epidemics, quarantines, other diseases, climatic aberrations, dangers incident to activities or from any other cause beyond The Travel Boutique’s control. (iii) Chargebacks from guests in the group; the group leader will be responsible.

I affirm that I am voluntarily participating in the travel services that The Travel Boutique has reserved on my behalf. In addition, The Travel Boutique is not responsible for special, indirect or consequential damages.  I hereby agree to indemnify and hold The Travel Boutique harmless from and against any and all claims, liabilities, and costs of any kind (including the costs of reasonable attorney’s fees and disbursements in defending such claims) to which they may be subject by reason of its negligence, the breach of this contract, or any other of its action. I understand that all reservation fares typically involve restrictions (including, but not limited to, restrictions related to cancellation) and that changing any aspect of my travel arrangements may result in the payment of additional money. I understand that The Travel Boutique only accepts payment via credit cards.  [Any advisor fees due to The Travel Boutique directly may be paid directly to The Travel Boutique in the form requested.]  By using a credit card, this may offer me the opportunity to dispute the charges should a vendor cease to operate and I am individually responsible for all actions required to submit such a dispute with my credit card provider. 

I understand that the Transportation Security Administration (TSA) requires me to carry a government-issued identification card in order to board a flight. I have been advised that the name, date of birth and gender that appears on the identification card must exactly match the same data that is listed on my airline ticket and in my booking records. I acknowledge that my failure to strictly comply with these requirements may result in denied boarding or an undue delay at an airport security checkpoint which may cause me to miss my flight. 

I understand that if traveling internationally, I must have a valid passport and depending upon my destination and nationality, I may need to obtain one or more visas. I take full responsibility and must determine the proper entry requirements when traveling internationally. This includes passports, visas, vaccinations, legal authorization, and any documents that may be required such as a minor traveling without parents.  All passengers are required to have a valid passport to travel. Passports must be valid for at least six months after the return date. Traveler assumes complete and full responsibility for, and hereby releases The Travel Boutique  from, any duty of checking and verifying any and all passport, visa, vaccination, or other entry requirements of each destination, and all safety and security conditions of such destinations, during the length of the proposed travel. It is the traveler’s responsibility to have proper passports or government-issued IDs where permitted / required. It is the traveler’s responsibility to be informed of any state department warnings where they are traveling. I also need to be aware of any health risks and take the appropriate precautions.

The Travel Boutique has no special knowledge regarding the financial condition of travel service providers selected for your travel, unsafe conditions, health hazards, weather hazards, or climate extremes at locations to which you may travel. For information concerning possible dangers at destinations, The Travel Boutique recommends contacting the Travel Warnings Section of the U.S. State Department at 1-888-407-4747 or www.travel.state.gov. For medical information, The Travel Boutique recommends contacting the Centers for Disease Control at 1-800-232-4636 or www.cdc.gov/travel. It is the traveler’s responsibility to protect their purchases and Travel Protection is strongly recommended.

Travelers are advised to obtain appropriate travel insurance coverage against these risks. By embarking upon his/her travel, the traveler voluntarily assumes all risks involved in such travel, whether expected or unexpected. The traveler is hereby warned of the above risks as well as possible travel industry bankruptcies and medical and climatic disruptions, and the possibility traveler may be unable to travel as scheduled because of personal emergency. Travel cancellation/interruption protection is available and highly recommended during or immediately after booking travel.

I will review my documents when I receive them and notify The Travel Boutique of any discrepancies before making payment on the invoice.  It is the traveler's responsibility to have appropriate documents for identification. It is the traveler's responsibility to read online or in the brochure from cruise lines, hotels, resorts, suppliers or tour companies and be aware of any cancellation penalties.  All monies paid for travel is subject to the refund / cancellation policy outlined below. Refunds, if due from suppliers; airlines, tour companies, resorts, cruise lines and hotels will be made to our customers directly from the supplier or, if applicable, the host agency.  The Travel Boutique is not responsible for any credits / refunds due from suppliers, nor can we control how and when they process credits / refunds.  Rates and availability are subject to change until a deposit / payment has been processed with the supplier.  [Once you have submitted your payment information, we will submit the payment to the supplier(s) and email you a receipt.  Until you receive a receipt from The Travel Boutique, your payment has not yet been processed.  We make every attempt to process payments the same day they are submitted.]  Cancellation may incur a full payment penalty.  Deposit may be non-refundable.  Checks are not accepted. 

Many airlines charge fees for checked luggage, check with your airline for additional charges.  Travel documents will be issued by Classic Vacations, Funjet Vacations, Apple Vacations, Blue Sky Tours, GoGo, Vacation Express, Delta Vacations, BedsOnline, Travel Impressions, or other suppliers. The Travel Boutique may book through a host agency. The Travel Boutique, therefore, shall not be responsible for breach of contract, failure to comply with any laws such as the Americans with Disabilities Act (ADA), or any intentional or negligent actions or omissions on the part of such suppliers, which result in any loss, damage, delay, inconvenience or injury to travelers or travelers' companions or group members.

Unless the term "guaranteed" is specifically stated in writing on your tickets, invoice, or reservation itinerary, or any other document from us, The Travel Boutique does not guarantee any of such supplier's rates, bookings, reservations, connections, scheduling, special requests, or handling of baggage or other personal effects. Travelers have done due diligence, are aware of the quality of the hotel accommodations chosen, and agree that The Travel Boutique will not be responsible if you are not satisfied.  While The Travel Boutique makes every effort to recommend only resorts / hotels that team members have stayed at personally, you should do your due diligence in approving / paying any and all proposals / invoices.  In the event that you are not completely satisfied with a resort / hotel upon arrival, please reach out to us so that we can attempt to resolve the issue with the suppliers directly.  I understand that The Travel Boutique and their Advisors may not be able to return my calls, schedule consults, or return emails until the evening after 5pm on weekdays. I agree that any action or proceeding brought by me against The Travel Boutique, their Travel Advisors or any host agency shall be brought solely in the courts of the city and state in which the Travel Advisor resides.  In the event any action or proceeding is initiated by me in a court outside of where the Travel Advisor lives, I agree to pay The Travel Boutique's costs and reasonable attorney fees associated with defending such action or proceeding.

Fees | Proposal Requests  |  The Travel Boutique provides up to three proposals, per trip, at no charge.  This includes changes to existing proposals you have received.  You will be invoiced for the proposal fee charge prior to us proceeding so that there are no hidden surprises.  While we would love to provide you with unlimited proposals, we invest a significant amount of time, resources and energy into putting together a proposal that will meet your specific travel needs and travel requests.  To help offset higher costs, we are unfortunately no longer able to provide more than 3 proposals [including changes] at no charge.  If you would prefer to have a consultation call before we build your first proposal, please schedule the call by visiting our Book a Consultation Appointment page.

Fees | After Booking

Changes | The Travel Boutique reserves the right to charge for changes to all reservations after the initial booking of said travel arrangements.  Each change made to a reservation, whatever the cause, will incur a $25.00 per person charge in addition to any supplier fees.  This includes name changes, traveler changes and removal of any services such as optional tours and transfers.  Please note that name changes to airline reservations are subject to full cancellation and rebooking.  Replacing a traveler constitutes a cancellation, is subject to cancellation penalties, and is not covered by this Changes Policy.  It is your responsibility to review all documents, upon receipt, to ensure the accuracy of your reservation.  The Travel Boutique does not assume any responsibility if you are denied any service by any supplier due to discrepancies, errors or missing information.  By agreeing to these Terms & Conditions, you agree that all of the traveler's information, such as, but not limited to the traveler’s name and dob have been provided accurately, matching their passport [for international travel] or other government issued identification [for domestic travel].  Again, additional fees charged by the supplier is also your responsibility and will be added to the invoice.  The result of incorrect information is your responsibility, including but not limited to a traveler being unable to board with the airline or other supplier.  We are NOT responsible for any errors or omissions nor any fees associated with necessary changes.  These charges, are not applied as a credit to the reservation balance, are non-refundable and not covered by travel protection. 

Cancellations | Cancellations will only be accepted in writing; email your cancellation request to us at cancellations@thetravelboutiquellc.com.  Travel protection plan payments and Advisor service fees are non-refundable and are typically non-transferable.  Deposits are typically non-refundable as well.  Please review your travel protection plan for coverage and exclusions.  

The Travel Boutique reserves the right to charge for cancellations once a reservation has been secured.  Cancellation fees will be charged, as follows, per reservation:

Again, these charges apply per reservation being canceled.  These charges will not be credited to any future reservations, are non-refundable and not covered by travel protection.  

Any fees charged by suppliers are also your responsibility and will be added to the invoice.  

Changes or additions after departure are subject to local rates at the time of amendment and are likely to be paid directly by the traveler to the supplier.

COVID-19 (CV-19)  |  The Travel Boutique is here to help guide you through the travel process.  We are not responsible for, nor do we guarantee, any outcomes.  We recommend that all travelers have a valid passport and adequate travel insurance coverage.

Due to the nature of the pandemic, all travelers are responsible for understanding what is required and for following those required guidelines.  You are also responsible for reading and understanding your travel protection plan coverage and exclusions.

The Travel Boutique is not responsible for travelers meeting any CV-19 vaccination requirements.  We are also not responsible for travelers knowing or understanding the latest news / policies regarding any CV-19 requirements.  We do require that all travelers be prepared for any changes that may occur without notice.

The Travel Boutique is not responsible for any charges you incur during travel due to any CV-19 requirements or occurrences.  We will do our best in advising you of the travel process and assist you with any new arrangements you may need to make during this time.  

COVID-19 Waiver of Liability | In Spring 2020, the World Health Organization (WHO) declared the Novel Coronavirus (COVID-19) a worldwide pandemic.  The Center for Disease Control (CDC) issued several preventative measures to combat the virus, such as frequent hand washing, wearing a mask or face covering and keeping at least 6 feet away from others.  If contracted, this virus has the ability to cause the traveler serious and severe illness.  For more information on the COVID-19 virus, please visit the CDC web page at www.cdc.gov. I understand and agree with the following:  1. COVID-19 is a highly contagious virus that can be spread via person to person contact;  2. After reasonable inquiry, The Travel Boutique provided me with the best available information regarding the pandemic-protective policies and practices of the suppliers used in my travel itinerary.  3. Supplier(s) that The Travel Boutique works with may have policies in place to help prevent the spread of COVID-19.  The Travel Boutique is not the drafter or holder of said policies and I understand that these policies can change at any time;  4. I understand that the supplier(s) may not apply those policies as diligently as the policies suggest and even if the supplier makes a good faith effort to enforce its good practices, some travelers may simply refuse to cooperate;  5. I understand that each state/country institutes their own pandemic protective policies and regulations.  I further understand that I am responsible for knowing, understanding and abiding by the pandemic protective policies and procedures regarding COVID-19 of all state(s)/ country(ies) listed in my travel itinerary, including any policy updates.  I also understand that I may be denied entry into the country for reasons within or outside of my control;  6. I understand that by traveling, I can be subject to voluntary or involuntary quarantine either at home or in the traveled location(s);  7. I am responsible for knowing, understanding and abiding by the pandemic protective policies and procedures regarding COVID-19 of all vendors used in my travel itinerary, including any policy updates;  8. With full awareness and appreciation of the risks involved, I, for myself, and on behalf of my traveling companions, including, but not limited to, my family, spouse, estate, heirs, executors, administrators, assigns, and personal representatives, assume all risks of travel and hereby forever hold harmless, waive, discharge, and release The Travel Boutique, its officers, advisors, independent contractors, affiliates, employees, successors, and assigns (collectively the “Released Parties”) from any and all liability, claims, demands, actions, and causes of action, directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me related to COVID-19 whether caused by the negligence of the Released Parties or any third-party supplier partnering with The Travel Boutique or any host agency.  9. I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all costs, expenses, damages, claims, lawsuits, judgments, losses, and/or liabilities (including attorney fees) arising either directly or indirectly from or related to any and all claims made by or against any of the Released Parties due to bodily injury or harm, death, loss of use, monetary loss, or any other injury from or related to my use of The Travel Boutique services, or the services of travel suppliers, specifically related to COVID-19.

While it our goal to exceed your travel expectations, The Travel Boutique does not guarantee ANY outcomes.


Late Payments  |  If your payment is due and you cannot pay on time, please contact us prior to your payment due date to potentially avoid fees and to see if alternative payment arrangements are available.

The above penalties do NOT include fees charged by the supplier or any other party.  Any fees or service charges from suppliers will also be automatically added to your invoice.

Pricing | All pricing listed is per person based upon double occupancy, unless otherwise clearly specified, and is subject to change at any time until the deposit or payment in full has been processed by the supplier.  [Once you have submitted your payment information, The Travel Boutique will submit the payment to the supplier and email you a receipt.  Until you receive a receipt from The Travel Boutique, your payment has not yet been processed with the supplier(s).  We make every attempt to process payments the same day they are submitted.  If you have not received a receipt within 24 hours, please reach out to us!]  

Pricing does NOT include items of a personal nature such as laundry, wines, water, beverages, food (some resorts may include certain items in their all-inclusive package or where clearly specified), passport and visa fees, insurance, foreign port taxes unless specifically indicated in the package inclusions or any resort or hotel fees that are paid at time of check in. Pricing is correct at time of publication; however, as airfares continually fluctuate and classes of service may have limited capacity, tour package prices and availability will change accordingly.

Travelers with Disabilities | In order to fully enjoy your travel services, we recommend that you select a trip that is suitable to your physical capabilities. Travelers requiring any form of assistance, including travelers with physical disabilities, sight or hearing impairments, are required to notify The Travel Boutique prior to reservation for review and agreement. Additionally, travelers must be accompanied by an individual responsible for providing needed services. Tour managers, guides, drivers or other tour, hotel, ship personnel are not able to provide such assistance. 

In order to participate in escorted tours, travelers must be able to understand and follow instructions given by the Tour Director at all times, both for the successful operation of the tour as well as for their personal safety. The Travel Boutique reserves the right to reject participation or remove any individual from certain travel services if notification was not provided and/or when, in our sole judgment, continued participation would significantly hinder the services to be provided to all travelers. Expenses, including cancellation fees and/or costs for alternate travel plans or to return home, will be the sole responsibility of the traveler. 

Escorted tours are fast-paced, often requiring lengthy walks over uneven terrain. In the interests of group harmony, travelers should be able to maintain the pace of the tour. Travelers with special needs may be better served independently rather than in a group setting. The Travel Boutique can suggest touring options based upon specific requirements. 

Wheelchairs & Walkers: Pursuant to the Americans with Disabilities Act (the ADA), The Travel Boutique seeks to accommodate travelers with disabilities to the fullest extent possible and consistent with the specific travel itinerary. Nevertheless, you may find that certain travel features may not be accessible to the extent that you require a wheelchair, scooter, or other special equipment to participate. We cannot provide individual assistance to travelers with wheelchairs or other mobility devices. We regret that some itineraries cannot accommodate wheelchairs or motorized scooters. 

Passengers are required to advise The Travel Boutique of their accessibility requirements prior to booking in order for The Travel Boutique to determine if reasonable accommodations are available. The Travel Boutique will endeavor to accommodate special access needs, but does not guarantee that we will be able to do so in all cases. 

International Travel: Resorts, hotels, sea and river cruises and other travel services outside of the United States are not required to comply with ADA requirements and therefore may not have ramps, wide entryways or elevators to accommodate travelers with disabilities or devices such as wheelchairs, walkers and motorized scooters. Due to physical constraints and space limitations, wheelchairs, walkers and motorized scooters may not be taken aboard motor coaches and river cruises. 

Service Animals: Service animals cannot be accommodated on international escorted tours. Passengers on USA escorted tours who require a service dog because of a disability should check with The Travel Boutique prior to booking a tour.

Health Requirements | Check with your healthcare provider for your up-to-date requirements. You may also check the Centers for Disease Control (www.cdc.gov) and/or the World Health Organization (http://www.who.int/en/) for their recommendations. Required inoculations, if any, must be recorded by the traveler’s health practitioner on a valid vaccination certificate which the traveler must carry for proof of inoculation where required. Individuals with heart disease, chronic illness, physical disability, advanced pregnancy, or mental illness should not participate in any rigorous travel programs.  Any person who arrives at the destination ill with an apparent fever or becomes ill during the tour, will be removed from the group and directed to a local medical facility for diagnosis.  Only upon clearance by an accredited medical facility will that individual be allowed to resume group travel. All costs associated with medical treatment and related expenses such as additional hotel nights or transportation is not included in the original itinerary and will be the responsibility of the traveler. 

Airline Frequent Flier Programs | Travelers are responsible for contacting their program airline directly regarding mileage eligibility and accrual. Airline frequent flier programs determine whether to award miles in part or total based on their own rules which are updated frequently.  Some discounted or promotional airfares as well as some codeshare flights are not eligible for mileage accrual.  Some private airfares are not eligible for mileage or may qualify for reduced mileage, even if the same airline class of service is eligible for full mileage when sold as an "Instant Purchase" published airfare.  Not all published airfares are eligible for mileage. The Travel Boutique will record frequent flier numbers when provided by the passenger prior to travel documents being issued. However, the addition of frequent flier numbers to airline records does not guarantee mileage eligibility which is at the sole discretion of each airline.  Airline schedule changes may result in flights which were originally eligible for mileage accrual no longer being eligible. We highly recommend travelers to provide their frequent flier account information whenever checking in online and/or at the airport check-in desk. After travel has commenced, it is often not possible to apply for frequent mileage credit. Travelers are also responsible to determine whether previously earned mileage may be applied to flights to secure upgrades. 

Contact Information: Pursuant to TSA Secure Flight requirements, travelers are responsible to provide The Travel Boutique with their name as it appears on their passport (or other government-issued ID when traveling domestically), date of birth, gender, address, phone number, email and fax data for all travelers. Travelers must ensure that names are correctly listed on their invoice. The Travel Boutique will not be responsible for travelers who do not receive an invoice or documents or are denied boarding due to inaccurate information.

Airline Name Changes | Any name change including minor spelling corrections may require airline reservations to be canceled and rebooked. Reservations are subject to current availability and pricing at the time of rebooking. Once airline tickets are issued, subsequent name corrections will be subject to an airline rebooking fee which may be as much as the full value of the airline ticket plus a $50 revision fee per change. The Travel Boutique is not responsible for the denial of services by a carrier due to any name discrepancy. Name changes must be advised in writing by emailing us at changes@thetravelboutiquellc.com.  Please refer to The Travel Boutique’s list of fees above for additional information on charges for name changes.

Schedule Changes | In the event of an airline schedule change, The Travel Boutique will make every effort to inform travelers of the schedule change and new flight schedule prior to departure.  The Travel Boutique, however, is not responsible for schedule changes including, when applicable, changes in routing and/or the number of stops in the itinerary. The Travel Boutique is unable to provide compensation for schedule changes, seat assignment modifications, or cancellations implemented by an airline.  In the event of any change in flight itinerary made directly between travelers and the airline, it is the traveler’s responsibility to advise The Travel Boutique of the amended flight details in writing by emailing us schedulechange@thetravelboutiquellc.com.  The Travel Boutique cannot be held responsible for land services, including arrival and/or departure transfers, if flights are changed without its knowledge.

Cruise Ship & Itinerary Changes | Cruise itineraries and ships are subject to change without notice. Furthermore, cruise ships may be chartered and/or departure dates canceled. The Travel Boutique accepts no responsibility for ship substitutions or itinerary changes imposed by a cruise line and is not responsible for any losses you may incur including the issuance and/or cancellation of airline tickets or visa fees.  

Medical Services: Many ships do not carry a doctor or nurse onboard.  Should medical attention be required, local services will be contacted.  Resulting charges will be the responsibility of the traveler. The Travel Boutique and the cruise ship operator are not responsible for the services provided.

Travel Services Agreement  |  I understand that The Travel Boutique is not the source or supplier of the travel services I have requested or purchased, and acts solely as an advisor for the actual suppliers of such services. I have been advised that the suppliers whose names appear in the information supplied to me are those who are actually responsible for providing the travel services I have requested or purchased.  I consent to and request the use of those suppliers and agree not to hold The Travel Boutique responsible should any of these suppliers:  1) fail to provide the travel services I have purchased;  2) fail to comply with any applicable law; or  3) engage in any negligent act or omission that causes me any sort of injury, damage, delay or inconvenience.

I will review my travel documents for accuracy upon receipt and understand that I may contact The Travel Boutique if I have any questions. I understand that discounted fares typically involve restrictions and that changing any aspect of my travel arrangements may result in additional charges.  Conversely, cancellation of any aspect of my travel arrangements may result in financial penalties, in accordance with The Travel Boutique’s policies as well as the supplier’s policies and may not be covered under any of the travel protection plans I may have purchased.

I have been advised that I must use a form of payment other than cash or check and that doing so may offer me the opportunity to dispute the charge should a supplier cease operation.

Privacy Notice  |  (TSA Update: August 13, 2009): The Transportation Security Administration (TSA) requires you to provide your full name, date of birth, and gender for the purpose of watch list screening, under the authority of 49 U.S.C section 114, the Intelligence reform and Terrorism prevention Act of 2004 and 49 C.F.F parts 1540 and 1560.  You may also provide your Redress Number, if available. I understand that the TSA requires me to carry a government issued identification card in order to board a flight.  I have been advised that the name, date of birth and gender that appears on my identification card must exactly match the same data that is listed on my airline ticket and in my booking records.  I acknowledge that my failure to strictly comply with these requirements may result in denied boarding or an undue delay at an airport security checkpoint and it may cause me to miss my flight.  I agree that neither The Travel Boutique or the suppliers used are responsible for any additional fees that I may incur for this reason.  

The Travel Boutique is not responsible for the issuance of travel identification such as passports and/or visas and that travelers should ensure they have or have been approved for the appropriate documents before purchasing travel. 

I understand that if traveling internationally, I must have a valid passport and depending upon my destination and nationality, I may need to obtain one or more visas.  I further understand that our passports must be valid for no less than 6 months from the travel date of return.

Note to Travelers  |  Passport and visa information may be obtained by contacting the Travel Advisory Section of the US State Department at 202.647.5225 or by visiting the State Department’s website at travel.state.gov.  Non-US passport holders should be sure to contact the embassies of their destination and transit countries to obtain entrance requirements.  To obtain medical information, you may contact the Centers for Disease Control at 404.332.4559 or visit the CDC’s website at www.cdc.gov

I understand that the airline tickets, air tours or other travel services that I am purchasing are subject to supplemental price increases that may be imposed after the original date of purchase. Post-purchase price increases may be applied due to additional costs imposed by a supplier or government.  I acknowledge that I may be charged additional sums by The Travel Boutique  or suppliers to offset increased fees, fuel surcharges, taxes, and fluctuations in foreign exchange markets or any combination thereof.  I hereby consent to any post-purchase price increases and authorize The Travel Boutique and suppliers to charge payment for such additional amounts after I have been notified of pricing change.  Post-Purchase price increases will be automatically added to your invoice and emailed to you.

In some locations, your aircraft may be subject to insecticide spraying prior to the flight or while you are on the aircraft.  You are strongly encouraged to always check the list of countries that require airlines to treat the passenger cabin with insecticides.   A regularly updated list can be found here:  http://alturl.com/3rtty.  

I understand travel protection plans have been offered to me for purchase which may cover certain risks inherent in travel such as supplier bankruptcy, trip interruption and/or the inability to travel due to a medical or personal emergency.  If I choose not to purchase a travel protection plan or if I purchase a travel protection plan on my own (not through The Travel Boutique), I understand that The Travel Boutique requires that I decline travel protection in writing. 

This is to confirm that, in keeping with all applicable laws, I am instructing the above Travel Advisor to purchase travel and/or travel protection against the form of payment instructed. 

I further understand that I have authorized this transaction and will hold The Travel Boutique and the suppliers harmless with respect to these instructions.  It is understood that The Travel Boutique and the suppliers may require government issued photo identification, a copy of the front & back of the card, and that The Travel Boutique and suppliers may verify the billing address of the card.

I will immediately review in detail any itinerary or invoice sent to me by The Travel Boutique.  And I agree that if any of the booking information is incorrect, including but not limited to: travel dates, itinerary schedule, airfare and/or car rental reservations, hotel, resort or cruise accommodations, optional activities and pricing, that I will immediately contact The Travel Boutique.  I understand that there may be change fees / other fees charged by The Travel Boutique and / or the suppliers and that I will be responsible for those additional fees.  I also understand that these fees will automatically be added to my invoice by The Travel Boutique, at the time of occurrence.  The Travel Boutique will email me with an updated invoice at each time of occurrence.

I agree that the spelling of all names listed (and the dates of birth, if shown) are correct and EXACTLY match the information as it reads on the passport or the government issued identification..

Hazardous Items  |  Federal law prohibits the carriage of certain hazardous items aboard the aircraft in your luggage and on your person and could result in a fine.  By agreeing with this document, by your continued use of the website, or use of The Travel Boutique services you acknowledge that you have been provided with the TSA Prohibited Items List (found here:  https://www.tsa.gov/travel/security-screening/whatcanibring/all-list) and that you understand you can be charged a fine if you are not in compliance.  You may view additional information at: http://www.faa.gov/about/initiatives/hazmat_safety/ or at http://safetravel.dot.gov/.

Photos [Any Media Related Material]  |  You hereby grant and authorize The Travel Boutique the right to take, edit, alter, copy, exhibit, phublish, distribute and make use of any and all pictures, videos or any other media related material taken of me / submitted by me, to be used in and/or for legally promotional materials including, but not limited to; newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. You agree that any pictures, videos, or other media related materials submitted to The Travel Boutique for use are in fact your own property and they do not infringe on any copyrights. This release, agreement and authorization extends to all languages, media, formats and markets now known or hereafter devised. This release, agreement and authorization shall continue indefinitely, unless I otherwise revoke said release, agreement and authorization in writing.  I understand and agree that these materials shall become the property of The Travel Boutique and will not be returned.

Reviews, Testimonials and/or Endorsements  |  You also agree and confirm that you have not been paid or offered any free services or products in exchange for the review, testimonial or endorsement.
