We encourage everyone to fill up this form regularly before we start to do our office work including those employees who are working from home. Filling up the said form and disclosing your symptoms and other necessary information (e.g. travel history) will help the management monitor your daily health status. Rest assured that the data you have submitted will be saved and kept as confidential by the Human Resource Management Office Database of Finance and Administrative Services Division (FASD) being the Administrator of this App. Lastly, we ensure that only the office Operating Unit Head and Section Heads of each Operating Unit can only view the data of their respective employees for their reference and monitoring.
