Assessment Request

1.upload the assessment request form per qualification in the google form provided 

2. Fill up the order of payment 2copies per candidate

3.Submit the Assessment request application together with the accomplished order of payment and 35 pesos processing fee

Reminder: The google form is intended only for walk in applicant if TESDA scholars no need na mag upload kay dili mn na cla apil sa processing fee for now until mareceived na nato ang clarificatory memo from CO
