Dear KUSD Parent/Guardian,

You have requested to checkout a KUSD Chromebook device from KUSD to help support your enrolled child with the expanding virtual learning opportunities starting in April, 2020. Please know that with checking out this device, you are agreeing to the liabilities related to expected use and care. Parents/Guardians electronically sign and agree to KUSD Policy 6633 Student Technology Acceptable Use Policy every year through the online registration requirement. Parents/Guardians are responsible for meeting all the expectations outlined in this policy and should abide by the guidelines set forth in this policy http://kusd.edu/sites/default/files/document-library/english/6633.pdf

1. Students are not allowed to modify their account rights or add an account to obtain additional access to their device.

2. Students are not allowed to modify the configuration of the hardware and software on their device.

3. Students must use their device solely for educational purposes, and their device is for their use only. KUSD will review reports of installed filtering/monitoring software to determine usage.

4. Students/parents must keep the KUSD username and password confidential.

5. Students must return the device undamaged and in working condition when requested.

A Chromebook guide sheet on proper care has been made available to you. Should the device be returned to the District in inoperable condition and/or damaged beyond normal use, the student and parent/guardian are responsible for the cost of repair or its fair market replacement value. If the device is lost or stolen, replacement expenses at the fair market value on the date of loss will be the responsibility of the student and parent/guardian. Depending on the model, the fair market replacement cost for the Chromebook will be between $216 and $275.

Failure to return the device to the District when requested to do so by the District may result in the District taking legal action for the return of its property. Should the District have to initiate any such proceedings, the parent/guardian and student will be responsible for the fees incurred by the District in obtaining the return of its property.
