Physical Form

The Tentative Physical Day for Paschal is May 6th! We will upload the new 2025-2026 Physical form and details for Physical Day as soon as we receive them! Check back soon!

This form is the only form that will be accepted for physicals. All cheerleaders will need a physical that is current through the tryout process. Current athletes Paschal and other FWISD schools may already have these forms completed for the school year. Please email me if you are not sure if yours are complete. Alejandra.Saenz2@fwisd.org.
