2025-2026 Physical Form and Medical History Form

This form is the only form that will be accepted for physicals. All cheerleaders will need a physical that is current through the tryout process. Current McLean athletes and other FWISD schools may already have these forms completed for the school year. Please email me if you are not sure if yours are complete. Melissa.Rogers@fwisd.org

Here are the instructions to log into Rank One. Please Note: If you are a student outside of the district, please email me and I will send you the paper version of the rankone forms. All other candidates from a FWISD school should be able to log in!
