Requirements to Participate in Soccer

*All forms due by November 30th (1st day of practice)

*Please give physical forms to Coach Medina or Coach Rivas.

1. Physical Completed & On File with Coaches. 

• Physical can be downloaded & printed off with the link provided or a hard copy can be picked up from the coaches or the front office at Kirkpatrick.  Once completed, the physical can be dropped off at the front office.

• Please complete the Medical Emergency Card and the Insurance & UIL Signature Page.
