In-Person Field Trip reservation and registration process
Complete the registration form to begin the process. A helpful tip is to review the form prior to clicking the submit button so that you have all of the necessary information to complete the form. Your progress can be saved and accessed at a later time.
The in-person field trip is not official until the Museum has received the ISD Partnership Registration Form.
The ISD can bring up to 120 students, teachers, and accompanying adult chaperones per tour date. One chaperone for every 10 students.
The ISD school groups should arrive by 10:45 a.m. for check-in and unloading. The in-person field trip start time is 11:00 a.m. ISD schools will depart the Museum no later than 1:30 p.m.
The lead teacher will receive a confirmation email with the reservation details after the completion of the Registration Form.
This confirmation email includes important logistical details for the in-person field trip information (arrival time, tour time, departure time, and field trip logistics), bus parking map, visiting guidelines, informational video, pre-visit resource guide, and other tour details.
The lead teacher MUST reply to the confirmation email to confirm receipt and provide the final attendance number to complete the reservation process.
The lead teacher for the ISD school will be required to submit a roster of ISD students and chaperones participating in the in-person field trip upon arrival at the Museum.

Transportation:
The field trip request forms are available online.
Teachers will collect their field trip forms from their campus Administrative Associate.
Complete all of the forms and return them to the campus Administrative Associate.
The Campus Administrative Associate will input the request into the field trip request system.
Once the field trip request is entered into the field trip request system, email all completed forms to Kathleen.hall1@fwisd.org and John.fernandez@fwisd.org.
