Write down the sport you are trying out for, the date, and the coach/staff in charge if you know.
Page 2 needs to be completed and RETURNED back to school prior/on the first day of tryouts.
Alternatively you can access EMA to see what athletics are being offered or speak to the ACLs of HPE.
Submitted a form on EMA or having the hard copy tryout form is required to tryout.

Download the form as a PDF file and fill out the pages.
Get this form signed by your parents and bring it in to the office to get the principal to sign.
Attach current driver's license and current vehicle insurance to this form before submitting to principal.
Forms must be submitted to the office 2 full days in advance for administration processing.
Once you receive the signed form from the office you must keep it in your vehicle when you are driving any other students.
