- SAVE AS PDF AND SEND BY EMAIL TO THE TEACHER/STAFF MEMBER THAT YOU WANT TO PREPARE THE LETTER

- Pick up PAPER COPY of the request form from your Guidance Counselor and submit directly to the teacher/staff member
