If your child is tardy, absent or dismissed due to a medical/dental/behavioral health appointment, please have the provider send in a note to excuse missed school time
If your child is absent- ill for 3 or more days, a medical provider note is needed to medically excuse the absences. In general, if a student is ill enough to miss school, and not improving on the 3rd day of symptoms, they should be evaluated by a medical provider for something more serious
Medical Provider notes that indicate a parent called the office to report student was absent due to illness, but student was not actually examined by a medical provider, are not accepted as medically excused absences.
Notes are also needed for a student to be medically excused from gym/recess and when those restrictions are no longer necessary (unless time frame was indicated on original note.)
