If you are a student from SYMS, VDM or SBSD you must submit the packet you received. If you did not receive a packet, you can pick one up in the SHYS Main Office. Please submit packet on the following dates.
Please submit packet at the SYHS Parent
Center during following date -
Morning Hours: 8:30am - 11:30am
March 14 (Tues): A - H
March 15 (Wed): I - P
March 16 (Thurs): Q- Z
April 5 -6 (Wed-Thurs): A-Z (late submission)
Afternoon Hours: 1pm- 3pm
April 5 (Wed): A-Z
April 6 (Thurs): A-Z (late submission)
Si es estudiante de SYMS, VDM o SBSD, debe traer el paquete que recibió. Si no recibió un paquete, puede recoger uno en la oficina principal de SHYS. Favor de traer el paquete en las siguientes fechas.
Entregue el paquete en el Centro de padres de SYHS durante las siguientes fechas y horarios -
8:30 am a 11:30 am
Marzo 14 (martes): A - H
Marzo 15 (miercoles): I - P
Marzo 16 (jueves): Q- Z
April 5 -6 (miercoles - jueves): A-Z (Entrega tardía)
1:00pm – 3:00pm
April 5 (miercoles): A-Z
April 20 (jueves): A-Z (Entrega tardía)
The packet includes:
1. Affidavit Verification of Residency
2. Online Registration Household Verification
3. Over-the-Counter (OTC) Medication Authorization Form
4. Emergency Authorization Card
5. Bring with you a copy of Proof Residency
6. Bring with you a copy of Immunization records
El paquete incluye:
1. Declaración Jurada Verificación de Residencia
2. Registro en línea Verificación del hogar
3. Formulario de autorización de medicamentos de venta libre (OTC)
4. Tarjeta de autorización de emergencia
5. Por favor traiga una copia de la Prueba de Residencia
6. Traiga consigo una copia de los registros de vacunación.
