How to Apply for DARC Services

• To be eligible to apply, you must contact the DARC Counselor. You can begin this process by completing the DARC Interest Form.

• If you have a verified disability and immediately would like to request accommodations, email the DARC Counselor to schedule an appointment at deborah.simone@sweetwaterschools.org  You will receive a response within 72 hours.  

• If you do not have a disability but may have experienced difficulty learning in the past and want further assistance.  Please complete the DARC Interest Form and you will receive a response within 72 hours.  

What is disability verification?

• Verification may include an old or current copy of your Individualized Education Plan (IEP)

• A copy of your 504 Plan from any high school, adult school, college or university

• If you have a medical, physical or mental health diagnosis, documentation must be completed by a health care professional or agency.
