REGISTRATION for NEW 7TH AND NEW 8TH GRADE 26-27

REQUIREMENTS:

1- Must live within Bonita Vista Middle School boundaries:

See School Site Locator for your designated school.
If you are NOT zoned to BVM, please follow transfer steps here.

2- Download Household form and Affidavit forms. Complete all sections on both sides of the form and save again to be able to submit electronically.

3- Upload COMPLETED documents here, please make sure to include the following:

OLR Household Form 2026-2027
Residency Affidavit 2026-2027
Proof of Residency
Immunization Records (and any other health inquiries)
Parent picture ID
IEP (if applicable)
504 Plan (if applicable)
Official Transcript (New 8th Graders ONLY)

4- Complete Online Registration Application (OLR 2026-2027)

Once all the forms above have been submitted you will receive an Activation Key for Campus Parent Portal.

Acceptable Proof of residence

o Utility Bill: Gas & Electric, Water, Cable TV, Internet, Trash or Sewer

o Verification of Social Services

o Lease Agreement/Rental Contract with Letterhead of the complex, business card from apartment manager and a current rental receipt

o Pay Stub
