REQUIREMENTS:
1- Must live in the Bonita Middle Area:
See School Site Locator for your designated school.
If you are NOT zoned to BVM, please follow transfer steps here.
2- Download Household form and Affidavit forms. Complete all sections on both sides of the form and save again to be able to submit electronically.
3- Upload all documents here, please make sure to include all the following:
2025-2026 Household Form
Affidavit of residency
Proof of residency
Immunization Records
Parent picture ID
IEP (if applicable)
504 Plan (if applicable)
transcript (only new 8th gr)
4- Complete Online registration
Once all the forms above have been submitted you will receive an Activation key for parent portal.

o Lease Agreement/Rental Contract with Letterhead of the complex, business card from apartment manager and a current rental receipt
