2- Download Household form and Affidavit forms. Complete all sections on both sides of the form and turn in to the front office at Bonita Vista Middle School.
3- Please be sure to provide the following:
OLR Household Form 2026-2027
Residency Affidavit 2026-2027
Proof of Residency
Immunization Records (and any other health inquiries)
Parent picture ID
IEP (if applicable)
504 Plan (if applicable)
Official Transcript (New 8th Graders ONLY)
4- Complete Online Registration Application (OLR 2026-2027)
Once all the forms above have been processed by the Registrar, you will receive a Welcome Letter Email with your Activation Key for Campus Parent Portal.
If you have a Campus Parent Portal account from previous years, sign in with your same username and password.
