In my private practice, I treat patients ages 14-65 and also provide LGBTQIA+-affirming care. I do not participate in one-time assessments geared towards completing forms required for medical clearance, disability claims, or legal proceedings. I do NOT currently accept medicaid/OHP health insurance.
I prefer that patients message me through the secure patient portal (https://swanpsychiatry.sessionshealth.com) or through email; I typically respond to messages within 24-48 hours.
Payment is due at the time of the appointment via credit/debit/HSA card through the online patient portal.
I am in network with Regence Blue Cross Blue Shield and Moda. I also accept cash pay and cannot provide superbills if you have health insurance not listed above.
I do not provide emergency psychiatric care for patients in my private practice. If you're having a mental health crisis immediately call 911 or the Multnomah County Crisis Line (503-988-4888, toll-free: 1-800-716-9679); or go to the nearest Emergency Department.
If you are a family member or a friend who has safety concerns for my patient, please do not wait for me to call back in case of an emergency. Call the non-emergency police line (503-823-3333) and request to have a welfare safety check for your loved one (prepare to have name, address, any contact info ready as well as your concerns) or call 911 if there are immediate safety concerns.
Patients are required to provide 24 hour advance notice for appointment cancellations. No-shows and cancellations within 24 hours will be charged $200. Please be aware that insurance does not cover any of these fees. Recurring missed appointments might result in removal from the practice.
Before requesting refills outside of appointments, please:
Check with your pharmacy to see if you have any refills remaining in their system.
If not, you can notify me through the patient portal at least 3 business days before running out of medication. Refill requests are not processed on weekends or holidays.
