We have placed a Behavior Record sheet in the prongs of each child's take home folder.
IF a teacher documents a behavior incident, it will be initialed and dated by the teacher with the details checked. Parents can see a daily record of how his/her day went. If marked by any teacher, the form must be signed and dated daily and shown to the teacher the following day.
The student is given a new slip on Monday to start with a clean slate. The teacher will keep the signed sheet in the student's file.
